Box 396 Hookerton, North Carolina 28538 AlcFn NCDENR Subject: Authorization to Construct ATC No. 025712A01 NPDES Permit No. NC0025712 Hookerton WWTP Greene County Dear Mayor. Purser: The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to Construct permit the Hookerton WWTP. The Division finds the proposed improvements to be satisfactory and hereby grants authorization for the following: • Installation of 146 linear feet of new 24" pipe with 24" "Tide Flex" check valve at discharge end of proposed piping. The only approved pipe materials for this project are either PVC (conforming to either ASTM standard F679 or D2680) or concrete (conforming to ASTM standard C76). • Fill of the existing effluent channel. Rip -rap shall be installed at a 3:1 side slope at each end of the channel. This Authorization to Construct is issued in accordance with NPDES Permit No. NC0025712, issued October 9, 2000, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the terms and conditions of the permit. Please also note that effluent sampling should be performed after all treatment units and prior to any dilution. In other words, the sample should not include storm water interference and include effluent from all treatment units. This is as per Part 11 Section D of your NPDES permit NC0025712 that states: "Samples collected and measurements taken, as required herein, shall be characteristic of the volume and nature of the permitted discharge....All samples shall be taken at the monitoring points specified in this permit and, unless otherwise specified, before the effluent joins of is diluted by any other wastestream, body of water, or substance." The Permittee shall notify the Water Quality Supervisor of the Washington Regional Office, telephone number (252) 946-6481, at least forty-eight (48) hours prior to operation of the installed facilities, so that an in -place inspection can be made. Notification shall be made during the normal office hours of 8:00 a.m. until 5:00 p.m., Monday through Friday. The Certification should be submitted to: NCDENR / DWQ, NPDES Unit, 1617 Mail Service Center, Raleigh, North Carolina 27699-1617. Requirements for Certified Operator The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of Title 15A, Chapter 8G, .0200. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays; and must properly manage and document daily operation and maintenance of the facility and comply with all other conditions of Title ISA, Chapter 8G, .0200. Additional Requirements The Operational Agreement between the Permittee and the Environmental Management Commission is incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms of the Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation of or failure to act in accordance with the terms and conditions of this Permit. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. The Permittee shall maintain a copy of the approved plans and specifications on file for the life of the facility. One (1) copy of the approved plans and specifications is enclosed for your records.
