4. Project Description and History
4a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:
The existing Bridge 34 in Martin County was built in 1955 and conveys traffic traveling NC 125/903 over Conoho Creek off the Roanoke River.Bridge 34 is a three span,33'wide x 141'
long steel girder bridge with 2-interior bents.The interior bents are supported by a 6.5'wide x 23'long x 2.5'thick reinforced concrete pile cap on timber foundation piles.The end bents are
armored with concrete slope protection.An underwater bridge inspection conducted on January 26,2021 indicated significant scour persists undermining the existing slope protection
causing it to break apart on the northeast corner of the bridge and crack in other places.The worst of the stream bed scour has or is occurring on the north side of the bridge(adjacent
Bent 2)as if the stream channel is aligning with that side of the creek.Bridge maintenance staff familiar with past conditions of the site indicate that the creek shoreline has eroded
landward back to the concrete slope protection on both sides of the bridge.The vertical scour against the toe of the slope protection varies from 0.5'to 4.0'and averages 2.5'and 0.9'
around the northern end bent and southern end bent,respectively.The horizontal scour under the concrete slope protection averages around 4'on each side of the bridge.The vertical
scour under the reinforced concrete foundation supporting the interior bents averages 1.4'below the bottom of the foundation on Bent 1 and Ton Bent 2.

4h.Explain the purpose of the proposed project:
The primary purpose of this project is to abate structural deficiencies with the bridge resulting from scour of the stream bed around both interior and end bents.The latest Structural Safety
Report generated from the January 26,2021 underwater inspection resulted in the issuance of a"Priority Action Request"(PAR)by the Division of Highways Structural Management Unit.
41.Describe the overall project in detail,including indirect impacts and the type of equipment to be used:*
Bridge maintenance staff proposes to address repair of the concrete slope protection,with the exception of the northeast corner where the slope protection has completely failed,by
attaching two layers of geotextile fabric to the top of the slope protection.The fabric will follow the slope protection to the toe of the slope.Bridge Maintenance will then install 2'x4'
sandbags at the toe of the slope protection on top of the fabric to keep the fabric in place,and to fill the voids under/around the toe of the slope protection.The sand bag revetment will
have two layers of bags for a vertical height of 4'.The bags will be filled with sand verse small rock to ensure flexibility in the bags to assist in sealing up the vertical void between the
stream bed and concrete slope protection.Maintenance staff will install fabric and sandbags just along the slope protection.The sandbags will hold the fabric in place while maintenance
staff pumps grout to fill all the voids under the slope protection.The pumping of grout to fill the voids will be done in phases with the intent of reducing the hydraulic pressure of the grout
filling the void and to prevent the grout from leaking through any remaining cracks in the concrete slope protection.A 24 hour period between pumping events will be followed to allow the
grout to setup after each pumping event.The northeast corner of the slope protection has completely failed.the remaining broken concrete will be removed and replaced with Class B
granite riprap.
To address the scour around the two interior bents maintenance staff would like the place#5 stone around the interior piles where the scouring is taking place.Maintenance staff plan to fill
the scour hole with#5 clean stone back to stream bed elevation,and to the bottom edge of the reinforced concrete foundation.A steel hopper filled with the#5 stone will be lowered to a
floating work barge.The work barge will then be positioned under a chain hoist to facilitate picking up the steel hopper and maneuvering it into position as directed a diver monitoring the
filling progress.PVC poles will be used to demark the location of the concrete foundation and outer edge of the scour hole.A diver will be present to monitor the filling and direct the
placement of the stone.The#5 stone will then be capped with Class B to prevent the#5 stone from being washed/scoured back out of the scour hole.

Utilizing sandbags provides for a smaller footprint than a sloping granite revetment and also provide a tighter seal to prevent the discharge of grout into
the stream.The rock plating of the stream bed will be limited to areas of scour.

The scour holes will be delineated with PVC poles and a diver will be used to assist in the fill levels in the scour holes.The pumping of grout into the
voids under the concrete slope protection will be done at a rates as low as possible.The grout pumping will be phased with 24 hours between pumping
events to allow the grout to setup.
