Attached are the NPDES inspection records from 5 -12 to 5 -19, including 3 inspections. The sediment loss at Y -2 occurred
on May 16, 2014. The loss was the result of a 1.724 rain event. Erosion control measures at the time of the event were
installed per plan. During the weekly inspection on May 12, no problems were noted at the SDO that drains the area of
the loss. As you saw during your site review on May 19, the off -site sediment has been removed, and the deposits in the
jurisdictional stream were removed to our satisfaction. The erosion control measures have been fortified and the area
has been mulched. We'll let you know if we have any more issues. Let me know if need anything else.

I believed we had silt loss on RP -D because of the amount of open grade along with the large amount of rain in a short
time span. These conditions overwhelmed the measures and caused the silt to go off the project. We have revisited the
area and mulched the open grade and fortified the measures.
