Tungsten Queen Mine Site — East Ravine
Dear Mr. Duckett:
On June 30, 2014, the Division of Water Resources (Division) received your application dated June
23, 2014, requesting a 401 Water Quality Certification from the Division for your project. The
Division has determined that your application is incomplete and cannot be processed. The
application is on -hold until the following information is received:
1. Since this project will result in greater than 150 linear feet of permanent stream impacts,
written approval for a 401 Water Quality Certification is required and must include the
appropriate fee. Please send a check for $570 to NCDWR; 401 & Buffer Permitting Unit;
1617 Mail Service Center; Raleigh, NC 27699 -1617.
2. Provide a plan for how the disturbed areas within the jurisdictional stream channel will be
restored and stabilized. Typical restoration activities include grading, installation of coir
matting and live stakes on the banks, and can also include grade control structures such as
cross veins.
Pursuant to Title 15A NCAC 02H .0502(c) and Title 15A NCAC 02B .0233 (8), the applicant shall
furnish all of the above requested information for the proper consideration of the application. If
all of the requested information is not received in writing within 30 calendar days of receipt of this
