Good afternoon Bruce,
I apologize for the delay in review time on this project — I was out of the office most of last week following a medical
procedure, so I lost nearly five days of work. Anyway, I've had a chance to review the Fast Track application and there
are a few things I'll need from you before I can step forward with permitting:
• Form FTA 06-21, Item V.3: It seems that items V.1 and V.2 were left blank because the new gravity line will
discharge directly to the existing pump station, but please include the general collections system number
(WQCS00078) and any name or number that is used to identify that pump station.
• Form FTSE 10-18, Items I.f, I.g, and I.h: These items must be filled in. Incomplete forms can't be accepted.
• Form FTSE 10-18, Item III: We will need a wet -ink signature or secure digital signature (such as a DocuSign
signature) on this form. Unfortunately no photocopies can be accepted.
Once you are able to resubmit these items, I should have a fairly quick turnaround to get the project permitted. Thanks
