(b) Continue to use the existing lift station that includes two-725 GIN pumps,two-300 GPP-4 pumps,and one- 16 GPM primary sludge pump. (c) Continue to use the existing primary clarifier(40 ft.dia.X 12 SW))). (d) Replaced the existing nine- 15 hp mechanical aerators in Lagoon No. 1 with a Biolac type diffused aeration system. (e) Relocated the existing 1,920 GPM Gorman Rupp pump from the existing Lightnin aeration system to Lagoon No. 1 for providing 2,000 GPI internal recirculation in Lagoon No. I. (1) Provided a new secondary clarifier(8)ft.dia.X 15 ft SWD). (g) Provided a new pump station that includes two •'14 GPM sludge recirculation pumps, two 200 GPM waste sludge/scum pumps,and two 700 GPM intermediate lift pumps. (h) Provided a flow meter and sampler for the influent to the Lagoon No. 1. (I) Provided a flow meter for waste sludge. Permission is also granted for the continued operation of the existing pharmaceutical wastewater collection and treatment facility(expanded by means stated above to a flow of'500,000 GPD)consisting of: (a)approximately 1,335 linear feet of eight-inch gravity sewer;approximately 1,600 linear feet of four-inch gravity drain line;approximately 705 linear feet of three-inch force main; a 240,000 gallon capacity aeration basin with two 20 horsepower platform aerators;a 40-foot by 12-foot clarifier for the pretreatment of process wastewater;a lift station consisting of two 725 GPM pumps and two 300 GPM pumps;a primary 16 GPM transfer pump;one 6.0 million gallon aerated storage lagoon,one 6.0 million gallon aerated basin, and a 36 million gallon lagoon; (b)transfer of evaporator condensate from the Company's plant facilities directly to the wastewater storage lagoon 1#2 without pretreatment in the aeration basin; (c)a 1,920 GPM Gonnan-Rupp centrifugal pump,six four-inch Mazzei injectors,and all the necessary valves and piping for the flow of wastewater to and from the injectors on the Lightnin basin, 1 06/04/2001 MON 11:01 FAX 919 494 3049 FINANCE/ACCOUNTING fa 005/012 (d)evaporative disposal of excess treated wastewater as necessary by recirculating water in lagoon#2 through the portable spray ini•ttion guns and back into the lagoons; (e)approximately n'ee feet of three-inch waste water collection piping to allow the collection of food preparation wastewater and the discharge of the collected wastewater into the existing process wastewater piping system, and the (f)spray inigation of treated wastewater on a total of 945.6 acres of available land as listed in condition VI 6.of this permit d including the following existing spray irrigation equipment: 1) a 700 GPM irrigation pump; 2) a 1500 GPM irrigation punip; 3) a 000 GPM pump; 4) one center pivot type irrigation system; 5) red type sprinkler equipment with portable spray guns;and 6) approximately 4,320 linear feet of six-inch diameter under gtou rid force main with hydrant connections; and approximately 1,375 linear feet of twelve-inch, 15,500 linear feet of eight-inch and 1,220 line 'feat of six-inch irrigation piping with hydrants. 7) The existing irrigation equipment per field is as follows: A) to the J ' ert al • sem* o ' s of t e E. n :-1,400 linear feet of six-inch inigation line,3,800 linear feet of eight-inch irrigation line,and twelve six-inch hydrants, B) to the Charles GgswjciçpppexfrS,400 linear feet of existing six-inch irrigation line ' ten hydrants, C) to the Novo Farm er2;two center pivot irrigation systems consisting of a Model :I• Valley Center Pivot system with a Nelson 1 end gun which provides 1,194 linear feet of wetted area and handles 450 GPM, 950 linear feet of six-inch irrigation pi.• 1.0 linear feet of eight-inch irrigation piping, and five six-inch hydrants; D) to the Novo Farm#3;fields•91/7 and W9(48.4 acres by center pivot)and a Model 8 ee0 Valley Center Pivot system with a Nelson 100 entigan which provides 670 linear feet of wetted area and handles 250 GPM to irrigate Farm 3,field G2(23.7 acres by center pivot),and 2,600 linear feat of six-inch inigation piping and seven six-inch hydrants, B) to the Novo Pmer#4;2,000 linear feet of six-inch irrigation piping,4,200 linear feet of eight-inch irrigation line with 19 hydrants,with the three areas to be irrigated by reel type sprinkler equipment, F) the William E.Peace Farm will be served by temporary, ••-..ve ground piping from the Novo Farm #2,Novo Farms#4 and the E.H.Yournigan Farm piping systems, and the The remaining acreage of these listed fields will be handled by portable irrigation equipment from existing irrigation piping. This permit is issued to insure proper operation of the above facility with no discharge of wastes to the surface waters,pursuant to the application receives June 20, 1997 and in conform:IV with the project plan,specifications,and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a pall of this permit. • This permit shall be effective from the date of issuance until April 30,2005,shall void Permit No. WQ0002806 issued March 6,2001,and shall be subject to the following specified conditions and limitations: T. PER ORMANCE STANDA The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily,including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater,the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 2 06/04/2001 MON 11:02 FAX 919 494 3048 FINANCE/ACCOUNTING 008/012 • 2. The issuance of this permit shall not relieve the Peri/ince of the responsibility for damages to sutfeee or groundwater resulting from the operation of this facility. 3. The residuals generated from these ettment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 4. Diversion or bypassing of the untreated Wastewater from the mann=facilities is prohibited. 5, The following buffers shall be maintained: a) 400 feet between wetted area and any residence or places of public assembly under separate ownership, b) 150 feet between wetted area and property lines, c) 100 feet between wetted area and wells, d) 100feer between wetted area and drainageways or surface water bodies, e) 50 feet between wetted area and public right of ways, f) 100 feet between wastewater treatment units and wells,and g) 50 feet between wastewater treatment units and property lines. Some of the buffers specified above may not have been included in previous permits for this waste treatment and disposal system. These buffers are not intended to prohibit or prevent modifications,which are seared by the Division, to improve performance of the existing treatment facility. These buffers do, • however, apply to modifications of the treatment and disposal facilities which are for the purpose of increasing the flow that is tributary to the facility. These buffers do apply to any expansion or modification of the spray irri ,doe areas and apply in instances in which the sale of property would cause any of the buffers now complied with, for the treatment and disposal facilities, to be violated. The Perminee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the facility must visit each Class T facility at least weekly and each Class II,ITT,and IV facility at least daily, excluding weekends and holidays. and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of 15A NCAC SA.0202 3. A suitable year round vegetative cover shall be maintained on all wastewater irrigation areas.Several acres of the land added with the most recent issuance of this permit had been cleared of trees.These areas shall have established et adequate vegetative cover over the fields and grassed waterways around the irrigation areas prior to initial irrigation on the fields.Agronomic rates(Plant Available Nitrogen)for the cover crop shall not be exceeded. 4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 5. If the TOC concentration as measured in the background monitor well exceeds 10 r g/I, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedanres of this naturally occurring TOC concentration in the downgredient wells shall be subject to the additional sampling and analysis as described above. The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring; Compliance Report Form)by the Groundwater Section, Permits and Compliance Unit, 1636 Mail Service Center,Raleigh,I .C.27699-1636 on or before the last working day of the month following the sampling month. 3. Any additional groundwater quality monitoring,as deemed necessary by the Division,shall be provided. 4. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system constmetcd after December 31, 1983 is established at either(1)250 feet from the waste disposal area, or (2)50 feet within the property bonndsary,whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate re-mediation action in addition to the penalty provisions applicable under General.-Statute 143- 215.6A(a)(1). • 5. In accordance with 1SA NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance • of standards at the Review Boundary shall require rceraediation action on the part of the puttee. V. INSPECTIONS • d Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment,a threat to human health,or a nuisance. The Permittee shall keep an inspection Jog or summary including as least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by.the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials,enter and inspect any property,premises or place on or related to the disposal site or facility at any reasonable time for the purpose of detemnining compliance with this permit;may inspect or copy any records that must be maintained under the terms and conditions of this permit,and may obtain samples of groundwater, surface-water,or leachate. This permit shall be effective from the date of issuance until April 30,2005,shall void Permit No.1NQ0002806 issued March 6,2001,and shall be subject to the following specified conditions and limitations: Io PERFORMANCE STANDARDS I. The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater,the Permitter shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. • 4, I. 4. 2 08/O4/2001 LION 11:02 FAX 919 494 3049 F./NANCE/ACCOUNTIl G r?ej 003/012 2. The issuance of this pelrmit shall not relieve the?mince of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 3. The residuals generated fro-., these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 4. Diversion or bypassing of the untreated Wastewater from the treatment facilities is prohibited, 5. The following buffers shall be maintained: a) 400 feet between wetted area and any residence or places of public assembly under separate ownership, b) 150 feet between wetted area and property lines, c) 100 feet between wetted area and wells, d) 100feet between wetted area and draistageways or surface water bodies, e) 50 feet between wetted area and public right of ways, f) 100 feet between wastewater treatment units and wells,and g) 50 feet between wastewater treatment units and property lines. Some of the buffers specified above may not have been included in previous permits for this waste treatment and disposal system. These buffers are not intended to prohibit or prevent modifications, which are requi - by the Division, to improve petfforrnance of the existing treatment facility. These buffers do, however, apply to modifications of the treatment and disposal facilities which are for the purpose of increasing the flow that is tributary to the facility. These buffers do apply to any expansion or modification of the spray irri.I tioo areas and apply in instances in which the sale of property would cause any of the buffers now complied with, for the treatment, and disposal facilities, to be violated. The applicant is advised that any modifications to the existing facilities will require a permit modification. IL 9PERAlIOI'I AND MAINTENANCE REO [tk:a.terENTs The facilities shall be properly maintained and operated at all times. 2. Upon classification of the facility by the Water Pollution Control System Operators Certification Commission (WPCSOCC),the Permittec shall employ a certified wastewater treatment plant operator to be in responsible charge(ORC)of the wastewater treatment facilities.The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the WPCSOCC. The Permiittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class(f,III,and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of 15A NCAC 8A.0202 3. A suitable year round vegetative cover shall be maintained on all wastewater irrigation areas.Several acres of the land added with the most recent issuance of this permit had been cleared of trees.These areas shall have established an adequate vegetative cover over the fields and grassed waterways around the irrigation areas prior to initial irrigation on the fields.Agronomic rates(Plant Available Nitrogen)for the cover crop shall not be exceeded. 4. Irrigation shall not be performed daring inclement weather or when the ground is in a condition that will cause runoff. 5. Adequate measures shall be taken to prevent wastewater runoff from the spray field. • 6. The application rate shall not exceed 156 inches per acre per year on Novoymes No a America Farms 1, 2,and 3,and fields VF-1,VF-2,and VF-3. 8. The application rate shall not exceed a cumulative loading of 73.8 inches over any twelve (12) month period on fields J-1,J-2,and I-3 and fields OF-1,GF-2,OF-3,and OF-4. 9. No type of wastewater other than that from Novoymes North America, Inc. shall be sprayed onto.the irrigation area. 10. No traffic or equipment shall he allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 11. Public access to the land application sites shall be controlled during active site ase. Such controls may include the posting of signs showing the activities being conducted at each site. 12. Several fields have rock outcrops within the spray areas. Please buffer and avoid irrigation on these rock outcrops. 13. If animal waste is being applied to any of the spray irrigation areas adequate protection shall be taken to insure this material is not washed off the site. Also, if animal waste is being applied to a field, the irrigation race shall be reduced to an appropriate level to insure tin t agronomic rates are not exceeded for that field. 14. Freeboard in the aeratio and storage lagoons shall not be less than one foot at any time. IIL MQPORTgEQRE5llNTs I. Any monitoring(inclueling groundwater,surface water.soil or plant tissue analyses)deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. Adequate records shall be maintained by the Permittee.tracking the amount of wastewater disposed. These records shall include,but are not necessarily limited to,the following information: a. date of irrigation, b. volume of waatewatcr irrigated, c. field irrigated, • d. length of time field is irrigated, e. continuous weekly,monthly,and year-to-date hydraulic(inches/acre)loadings for each field, f. weather conditions,and g. maintenance of cover crops. 3. Three (3)copies-of all operation and disposal records (as specified in condition 111 2) on Form NDAR-I shall be reported uonthly and submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data(as specified in condition III 3) on Form NDIYIR-1 shall be reported monthly and submitted on or before the last day of April,August,and December. Noncomplience Notification: The Pennines shall report by telephone to the Raleigh Regional Office, telephone number(919)571-4700 as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: • a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester;the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known. or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors,etc. c. Any failure of a pumping station, iewer line., or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. Any time that self-monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report nest outline the actions taken or proposed to be taken to ensure that the problem does not recur. rv. G.ROUNDWATER REOUDR.EMENTS 1. For purposes of reporting data, the following changes shall be made regarding the identities of the monitoring wells: e4W-4A will be reported as MW-il; MW-7A will be reported as MW-2; MW-SA will be reported as Iv1W-3; MW-12A will be reported as MW-12;and MW-I6A will be reported as MW-16. 2. The=depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. • TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this 'roc concentration. if the TOC concentration as measured in the background monitor well exceeds 10 mg11, this concentration will be taken to represent the naturally occurring TOC concentration. Any eaiceeda nces of this naturally occurring TOt concentration in the downgradicnt wells shall be subject to the a. Wane) sampling and analysis as described above. • The results of the sampling and analysis must be received on Form CiW-59 (Groundwater Quality Monitoring: Compliance Report Form)by the Groundwater Section, Permits and Compliance Unit, 1636 Mail Service Center,Raleigh,INC.27699-I636 on or before the last working day of the month following the sampling month_ 3. Any additional groundwater quality monitoring,as deemed necessary by the Division,shall be provided. 4. The COMPLIANCE,BOUNDARY for the disposal system is specified by regulations in 15A NCAC Groundwater Classifications and Standards. The Compliance Boundary is for the ;'<posal system constructed after December 31, 1983 is established at either(1)250 feet from the waste disposal area,or (2)50 feet within the property boundary,whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under mil Statute 143- 215.6A(a)(1). 5_ In accordance with I5A NCAC 2L,a,REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the permittee. V. INSPECTIONS I. Adequate inspection, maintenance, and cleaning shall be provided by the Penuinee to insure proper operation of the subject facilities. 2. The Pentnittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment,a threat to hum health,or a nuisance. The Permittee shall keep an inspection log or sununary including at least the date and rime of inspection, observations made, and any maintenance, repairs, or corrective actions taken by.the Pcrraittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. Any duly-authorized officer, employee, or representative of the Division may, upon presentation of credentials,enter and inspect any property,premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit;may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water,or leachate. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit,the approved plans and specifications,and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data.. 3. This permit is not transferable. In the event there is a desire for the facilities to change ownership,or there is a name change of the Permittee,a formal permit request must be submitted to the Division accompanied by an application fee,documentation from the parties involved,and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. • 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an • enforcement action by the Division in accordance with North Carolina General Statute 143.2I5.6A to 143- 215.6C. 5. The issuance of this permit does not preclude the Permittee from complying..with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal)which have jurisdiction. 6.
