3. Is being effectively operated or will,when completed,be required to operate in accordance with the terms and conditions of the permit,certificate of approval, or other document of approval
issued by the DWR;and
4. Has or,when completed,will have as its PRIMARY rather than incidental purpose the reduction of water pollution;
5. Is being used exclusively(100%)for waste treatment.

Land: To qualify for tax certification,the land must be used exclusively for water pollution prevention. A schematic diagram of the facility detailing the land being applied for is required. The amount of land
(acreage)is required and how this land is being used to prevent water pollution. For wastewater treatment and irrigation system list separately the acreage used for actual treatment from the acreage required
by the facility's water quality permit for buffers and setbacks.

SPRAY IRRIGATION SYSTEM PERMIT
In accordance with the provisions of Article 21 of Chapter 143,General Statutes of North Carolina as amended,and
other applicable Laws,Rules,and Regulations
PERMISSION IS HEREBY GRANTED TO
Novozymes North America, Ins.
Franklin County
FOR THE
continued operation of a pharmaceutical wastewater treatment plant expanded to 500,000 GPD by the previous
modifications as follows:
(a) The existing 240,000 gallon capacity Ligh coin aeration system is used as back up for providing treatment of
permeate wastewater flow.
(b) Continue to use the existing lift station that includes two-725 GPM pumps,two-300 GPM pumps,and one-
16 GPM primary sludge pump.
(c) Continue to use the existing primary clarifier(40 ft.dia.X 12 S WD).
(d) Replaced the existing nine- 15 hp mechanical aerators in Lagoon No. 1 with a Biolac type diffused aeration
system.
(e) Relocated the existing 1,920 GPM Gorman Rupp pump from the existing Lightnin aeration system to Lagoon
No. I for providing 2,000 GPM internal recirculation in Lagoon No. 1.
(f) Provided a new secondary clarifier(80 ft.dia.X 15 ft S WD).
(g) Provided a new pump station that includes two 400 GPM sludge recirculation pumps, two 200 GPM waste
sludge/scum pumps,and two 700 GPM intermediate lift pumps.
(h) Provided a flow meter and sampler for the influent to the Lagoon No. l.
(i) Provided a flow meter for waste sludge.
Permission is also granted for the continued operation of the existing pharmaceutical wastewater collection and .
treatment facility(expanded by means stated above to a flow of 500,000 GPD)consisting of:
(a)approximately 1,335 linear feet of eight-inch gravity sewer;approximately 1,600 linear feet of four-inch
gravity drain line;approximately 705 linear feet of three-inch force main; a 240,000 gallon capacity aeration
basin with two 20 horsepower platform aerators; a 40-foot by 12-foot clarifier for the pretreatment of process
wastewater;a lift station consisting of two 725 GPM pumps and two 300 GPM pumps;a primary 16 GPM
transfer pump; one 6.0 million gallon aerated storage lagoon,one 6.0 million gallon aerated basin, and a 36
million gallon lagoon;
(h)transfer of evaporator condensate from the Company's plant facilities directly to the wastewater storage lagoon
!l2 without pretreatment in the aeration basin;
(c)a 1,920 GPM Gorman-Rupp centrifugal pump.six four-inch Mazzei injectors,and all the neceqsary valves and
piping for the flow of wastewater to and from the injectors on the Lightnin basin,

(d)evaporative disposal of excess treated wastewater as necessary by recirculating water in lagoon#2 through the
portable spray irrigation guns and back into the lagoons;
(e)approximately nine feet of three-inch waste water collection piping to allow the collection of food preparation
wastewater and the discharge of the collected wastewater into the existing process wastewater piping system,
and the
(f)spray irrigation of treated wastewater on a total of 945.6 acres of available land as listed in condition VI.6.of
this permit and including the following existing spray irrigation equipment:
1) a 700 GPM irrigation pump;
2) a 1500 GPM irrigation pump;
3) a 800 GPM pump;
4) one center pivot type irrigation system;
5) reel type sprinkler equipment with portable spray guns; and
6) approximately 4,320 linear feet of six-inch diameter underground force main with hydrant connections;
and approximately 1,375linear feet of twelve-inch, 15,500 linear feet of eight-inch and 1,220 linear feet
of six-inch irrigation piping with hydrants.
7) The existing irrigation equipment per field is as follows:
A) to the Rel.Joumiaan property(also will servine oorliona of the E.1 leurnigan Furrn):1,400 linear
feet of six-inch irrigation line,3,800 linear feet of eight-inch irrigation line,and twelve six-inch
hydrants,
B) to the Charles Goswickpropeety-5,400 linear feet of existing six-inch irrigation line with ten hydrants,
C) to the Novo Farm#2; two center pivot irrigation systems consisting of a Model 8000 Valley Center
Pivot system with a Nelson 100 end gun which provides 1,194 linear feet of wetted area and handles
450 GPM, 950 linear feet of six-inch irrigation piping, 10 linear feet of eight-inch irrigation piping,
and five six-inch hydrants,
D) to the Novo Farm#3;fields W7 and W9(48.4 acres by center pivot)and a Model 8000 Valley Center
Pivot system with a Nelson 100 end gun which provides 670 linear feet of wetted area and handles 250
GPM to irrigate Farm 3,field 02(23.7 acres by center pivot),and 2,600 linear feet of six-inch
irrigation piping and seven six-inch hydrants,
F,) to the Novo Farm#4;2,000 linear feet of six-inch irrigation piping,4,200 linear feet of eight-inch
irrigation line with 19 hydrants,with the three areas to be irrigated by reel type sprinkler equipment,
F) the William E.Pearce Pam will be served by temporary,above ground piping from the Novo Farm
42,Novo Farms#4 and the L.H.Joumigan Farm piping systems, and the
The remaining acreage of these listed fields will be handled by portable irrigation equipment from
existing irrigation piping.
This permit is issued to insure proper operation of the above facility with no discharge of wastes to the surface
waters,pursuant to the application received June 20, 1997 and in conformity with the project plan,specifications,and
other supporting data subsequently filed and approved by the Department of Environment, Health and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until April 30, 2005, shall void Permit No. WQ0002806
issued March 6,2001,and shall be subject to the following specified conditions and limitations:
T. PERFORMANCE STANDARDS
. The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no
discharge to the surface waters, nor any contamination of ground waters which will render them
unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater,the
Permittee shall take immediate corrective actions including those actions that may be required by the
Division of Water Quality (Division), such as the construction of additional or replacement wastewater
treatment and disposal facilities.

2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or
groundwaters resulting from the operation of this facility.
3, The residuals generated from these treatment facilities must be disposed in accordance with General
Statute 143-215.1 and in a manner approved by the Division.
4. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.
5. The following buffers shall be maintained:
a) 400 feet between wetted area and any residence or places of public assembly under separate
ownership.
b) 150 feet between wetted area and property lines,
c) 100 feet between wetted area and wells,
d) 100 feet between wetted area and drainageways or surface water bodies,
e) 50 feet between wetted area and public right of ways,
f) 100 feet between wastewater treatment units and wells,and
g) 50 feet between wastewater treatment units and property lines.
Some of the buffers specified above may not have been included in previous permits for this waste
treatment and disposal system. These buffers are not intended to prohibit or prevent modifications, which
are required by the Division, to improve performance of the existing treatment facility. These buffers do,
however, apply to modifications of the treatment and disposal facilities which are for the purpose of
increasing the flow that is tributary to the facility. These buffers do apply to any expansion or
modification of the spray irrigation areas and apply in instances in which the sale of property would cause
any of the buffers now complied with, for the treatment and disposal facilities, to be violated. The
applicant is advised that any modifications to the existing facilities will require a permit modification.
II. QPIERNIION AND MAINTENANCE REOUYREMENTS
• 1. The facilities shall be properly maintained and operated at all times.
2. Upon classification of the facility by the Water Pollution Control System Operators Certification
Commission (WPCSOCC),the Petmittee shall employ a certified wastewater treatment plant operator to
be in responsible charge(ORC)of the wastewater treatment facilities.The operator must hold a certificate
of the type and grade at least equivalent to or greater than the tJassification assigned to the wastewater
treatment facilities by the WPCSOCC. The Permitter must also employ a certified back-up operator of the
appropriate type and Bade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the
facility must visit each Class T facility at least weekly and each Class U, III,and IV facility at least daily,
excluding weekends and holidays, and must properly manage and document daily operation and
maintenance of the facility and must comply with all other conditions of 15A NCAC 8A.0202
3. A suitable year round vegetative cover shall be maintained on all wastewater irrigation areas.Several acres
of the land added with the most recent issuance of this permit had been cleared of trees.These areas shall
have established an adequate vegetative cover over the fields and grassed waterways around the irrigation
areas prior to initial irrigation on the fields.Agronomic rates(Plant Available Nitrogen)for the cover c,op
shall not be exceeded.
4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will
cause runoff.
5. Adequate measures shall be taken to prevent wastewater runoff from the spray field.
6. The facilities shall be effectively maintained and operated as a non-discharge system to prevent the
discharge of any wastewater resulting from the operation of this facility.

7. The application rate shall not exceed 156 inches per acre per year on Novoymes North America Farts 1,
2,and 3,and fields VF 1,VF 2,and VF-3.
9. - The application rate shall not exceed a cumulative loading of 73.8 inches over any twelve (12) month
period on fields 3-1,1-2,and J-3 and fields GF 1,OF-2,GF 3,and GF-4.
9, No type of wastewater other than that from Novoymes North America, Inc. shall be sprayed onto the
irrigation area.
10. No traffic or equipment shall he allowed on the disposal area except while installation occurs or while
normal maintenance is being performed.
11. Public access to the land application sites shall be controlled during active site use. Such controls may
include the posting of signs showing the activities being conducted at each site.
12. Several fields have rock outcrops within the spray areas. Please buffer and avoid irrigation on these rock
outcrops.
13. If animal waste is being applied to any of the spray irrigation areas adequate protection shall be taken to
insure this material is not washed off the site. Also, if animal waste is being applied to a field, the
irrigation rate shall be reduced to an appropriate level to insure that agronomic rates are not exceeded for
that field.
14. Freeboard in the aeration and storage lagoons shall not be less than one foot at any time,
II1. MON TORIN_G_AND REPORTING REQUIREMENTS
I. Any monitoring(including groundwater,surface water,soil or plant tissue analyses)deemed necessary by
the Division to insure surface and ground water protection will be established and an acceptable sampling
reporting schedule shall be followed.
2. Adequate records shall be maintained by the Pennittee tracking the amount of wastewater disposed. These
records shall include,but are not necessarily Iimited to,the following information:
a date of irrigation,
b. volume of wastewater irrigated,
c. field irrigated,
d. length of time field is irrigated,
e. continuous weekly,monthly,and year-to-date hydraulic(inches/acre)loadings for each field,
f. weather conditions,and
g. maintenance of cover crops.
3. The effluent from the subject facilities shall be monitored by the Permittee at the point prior to irrigation
every March,July,and November for the following parameters:

shall be reported monthly and submitted on or before the last day of the following month. Three (3)
copies of all effluent monitoring data(as specified in condition III 3)on Form NDMR-1 shall be reported
monthly and submitted on or before the last day of April,August,and December. All information shall be
submitted to the following address:

The Perinittee shall report by telephone to the Raleigh Regional Office,telephone number(919)571-4700
as soon as possible, but in no case more than 24 hours or on the next working day following the
occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of significant
amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the
contents of a sludge digester;the known passage of a slug of hazardous substance through the facility;
or any other unusual circumstances.
b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of
adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,
compressors,etc.
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or facility.
d. Any time that self-monitoring information indicates that the facility has gone out of compliance with
its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter form within five
(5) days following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur.
Iv. GR0IJJO)WATER REOVIREMENTS
ENTS
1. For purposes of reporting data, the following changes shall be made regarding the identities of the
monitoring wells:
MW-4A will be reported as MW-l;
MW-7A will be reported as MW-2;
MW-SA will be reported as MW-3;

2, Existing monitor wells MW-i, MW-2, MW-3, MW-11, MW-12, MW 13, MW-14, MW-15, MW-16,
MW-17, MW-18, MW-19, MW-20, MW-21, MW-22, MW-23, MW-24, and MW-25 shall be sampled
every March,July and November for the following parameters:

The measurement of water levels must be made prior to sampling for the remaining parameters. The depth
to water in each well shall be measured from the surveyed point on the top of the casing.
The measuring points(top of well casing)of all monitoring wells shall be surveyed to provide the relative
elevation of the measuring point for each monitoring well.
If TOC concentrations greater than 10 rng11 are detected in any downgradieut monitoring well, additional
sampling and analysis must be conducted to identify the individual constituents comprising this 'TOC
concentration. If the 'TOC concentration as measured in the background monitor well exceeds 10 mg/1,
this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedanoes
of this naturally occurring TOC concentration in the downgradicnt wells shall be subject to the additional
sampling and analysis as described above.
The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality
Monitoring: Compliance Report Form)by the Groundwater Section, Permits and Compliance Unit, 1636
Mail Service Center,Raleigh,N.C.27699-1636 on or before the last working day of the month following
the sampling month.
3. Any additional groundwater quality monitoring as deemed necessary by the Division,shall be provided.
4. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L,
Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system
constructed after December 31, 1983 is established at either(1)250 feet from the waste disposal area, or
(2)50 feet within the property bon/Atty.whichever is closest to the waste disposal area. An exceedance
of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate
remediation action in addition to the penalty provisions applicable under General. Statute 143-
2I5.6A(a)(1).
5. In accordance with ISA NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems
midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance
of standards at the Review Boundary shall require rcmediation action on the part of the permittee.
V. INSPECTIONS
I. Adequate inspection, maintenance, and cleaning shall be provided by the Perrnittee to insure proper
operation of the subject facilities.
2. The Permdttee or his designee shall inspect the wastewater treatment and disposal facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of
wastes to the environment,a threat to human health,or a nuisance. The Permittee shall keep en inspection
log or summary including at least the date and time of inspection, observations made, and any
maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be
maintained by the Permittee for a period of three years from the date of the inspection and shall be made
available upon request to the Division or other permitting authority.
3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of
credentials,enter and inspect any property,premises or place on or related to the disposal site or facility at
any reasonable time for the purpose of determining compliance with this permit;may inspect or copy any
records that must be maintained under the terms and conditions of this permit,and may obtain samples of
groundwater,surface water,or leachate.

appropriate. The approval of this request will be considered on its merits and may or may not be
approved.
4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to en
enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
5. The issuance of this permit does not preclude the Pernuttee from complying.with any and all statutes,
toles, regulations, or ordinances which may be imposed by other government agencies (local, state, and
federal)which have jurisdiction.

the continued operation of the abject wastewater treatment and spray irrigation facilities.
This permit shall be effective from the date of issuance until April 30, 2005, shall void Permit No.
WQ0002806 issued May 15, 2001,and shall be subject to the conditions and limitations as specified therein.
Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate
system for collecting and maintaining the required operational information will result in future compliance
problems.

Please note that Item (d) of the first part of the system description has been updated to reflect that
NNBNA decided not to relocate the mechanical aerators to Lagoon 2. Item (a) of the second part of the
system description has been amended to reflect that one of the two previously permitted 6.0 million-gallon.
aerated storage lagoons is now serving as an aeration basin.
Condition M. 4. has been amended to require that monitoring be reported monthly on both Form
NDMR and Form NDAR.
Please be aware that some of the permit conditions in Section IV. Groundwater Requirements have
changed.Permit Condition VI. 1.recommends the renaming of several of the monitoring wells for purposes of
reporting monitoring results. In lieu of reported trends of increasing concentrations of Total Ammonia and •
Nitrate Nitrogen in many of the wells,Permit Condition VT.2.requires that the monitoring wells be sampled
for Ammonia Nitrogen instead of Total Ammonia as has been previously required.
If any pans, requirements, or limitations contained in this permit are unacceptable,you have the right
to request an adjudicatory hearing upon written request within thirty(30)days following receipt of this permit.

PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of a pharmaceutical wastewater treatment plant expanded to 500,000 GPD by the previous
modifications as follows:
(a) The existing 240,000 gallon capacity Lightnin aeration system is used as back up for providing treatment of
permeate wastewater flow.
(b) Continue to use the existing lift station that includes two-725 GPM pumps,two. 300 GPM pumps,and one-
16 GPM primary sludge pump.
(c) Continue to use the existing primary clarifier(40 ft.dia.X 12 SWD).
(d) Replaced the existing nine- 15 hp mechanical aerators in Lagoon No. 1 with a Biolac type diffused aeration
system.
(e) Relocated the existing 1,920 GPM Gorman Rupp pump from the existing Lightnin aeration system to Lagoon
No.1 for providing 2.000 GPM internal recirculation in Lagoon No. 1.
(f) Provided a now secondary clarifier(80 ft.dia.X 15 ft SWi)).
(g) Provided a new pump station that,includes two 400 GPM sludge recirculation pumps, two 200 GPM waste
sludge/scum pumps,and two 700 GPM intermediate lift pumps.
(h) Provided a flow meter and sampler for the influent to the Lagoon No. 1.
(i) Provided a flow meter for waste sludge.
Permission is also granted for the continued operation of the existing pharmaceutical wastewater collection and -
treatment facility(expanded by means stated above to a flow of 500,000 GPD)consisting of:
(a)approximately 1,335 linear feet of eight-inch gravity sewer;approximately 1,600 linear feet of four-inch
gravity drain line;approximately 705 linear feet of three-inch force main; a 240,000 gallon capacity aeration
basin with two 20 horsepower platform aerators; a 40-foot by 12-foot clarifier for the pretreatment of process
wastewater;a lift station consisting of two 725 GPM pumps and two 300 GPM pumps;a primary 16 GPM
transfer pump; one 6.0 million gallon aerated storage lagoon,one 6.0 million gallon aerated basin, and a 36
million gallon lagoon;
(b)transfer of evaporator condensate from the Company's plant facilities directly to the wastewater storage Iagoon
#2 without pretreatment in the aeration basin;
(c)a 1,920 GPM Gorman-Rupp centrifugal pump,six four-inch Mazzei injectors,and all the necessary valves and
piping for the flow of wastewater to and from the injectors on the Lightnin basin,

(d)evaporative disposal of excess treated wastewater as necessary by recirculating water in lagoon#2 through the
portable spray irrigation guns and back into the lagoons;
(e)approximately nine feet of three-inch waste water collection piping to allow the collection of food preparation
wastewater and the discharge of the collected wastewater into the existing process wastewater piping system,
and the
(f)spray irrigation of treated wastewater on a total of 945.6 acres of available land as listed in condition VL 6.of
'tie permit and including the following existing spray irrigation equipment:
1) a 700 GPM irrigadon pump;
2) a 1500 GPM irrigation pump;
3) a 800 GPM pump;
4) one center pivot type irrigation system;
5) reel type sprinklerequipment with portable spray guns;and
6) approximately 4,320 linear feet of six-inch diameter underground force main with hydrant connections;
and approximately 1,375 linear feet of twelve-inch, 15,500 linear feet of eight-inch and 1,220 linear feet
of six-inch irrigation piping with hydrants.
7) The existing irrigation equipment per field is as follows:
A) to the ILT.Ionmigan prestenTialsnieill serving portions of the Ellicattnigan Farm)-1.400 linear
feet of six-inch irrigation line.3,800 linear feet of eight-inch irrigation line,and twelve six-inch
hydrants,
B) to the Pharles Goswick property-5,400 linear feet of existing six-inch irrigation line with ten hydrants,
C) to the?Tow"Farm 02;two center pivot irrigation systems consisting da Model 8000 Valley Center
Pivot system with a Nelson 100 end gun which provides 1,194 linear feet of wetted area and handles
450 GPM. 950 linear feet of six-inch irrigation piping, 10 linear feet of eight4ich irrigation piping,
and five six-inch hydrants,
D) to the Novo Farm#3;fields W7 and W9(48.4 acres by center pivot)and a Model 8000 Valley Center
Pivot system with a Nelson 100 end gun which provides 670!blear feet of wetted area and handles 250
GPM to irrigate Farm.3,field 02(23.7 acres by center pivot),and 2,600 linear feet of six-inch
irrigation piping and seven sit-inch hydrants,
E) to the Novo Farm#4;2,000 linear feet of six-inch irrigation piping,4,200 linear feet of eight-inch
irrigation line With 19 hydrants,with the three areas to be irrigated by reel type sprinkler equipment,
F) the William E.Pearce Farm will be served by temporary,above ground piping from the Nuvo Farm
#2,Novo Farms#4 and the E.H.Journigan Farm piping systems, and the
The remaining acreage of these listed fields will be handled by portable irrigation equipment from
existing irrigation piping.
This permit is issued to insure proper operation of the above facility with no discharge of wastes to the surface
waters,pursuant to the application received June 20, 1997 and in conformity with the project plan,specifications,and
other supporting data subsequently filed and approved by the Department of Environment, Health and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until April 30, 2005,shall void Permit No.WQ0002806
issued March 6,2001,and shall be subject to the following specified conditions and limitations:
I. PERFORMANCE STANDARDS
1, The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no
discharge to the surface waters, nor any contamination of ground waters which will render them
unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the
creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater,the
Permittee shall take immediate corrective actions including those actions that may be required by the
Division of Water Quality (Division), such as the construction of additional or replacement wastewater
treatment and disposal facilities.

2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or
groundwaters resulting from the operation of this facility.
3. The residuals generated from these treatment facilities must be disposed in accordance with General
Statute 143-215.1 and in a manner approved by the Division.
4. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.
5. The following buffers shall be maintained:
a) 400 feet between wetted area and any residence or places of public assembly under separate
ownership,
b) 150 feet between wetted area and property lines,
c) 100 feet between wetted area and wells,
d) 100 feet between wetted area and drainageways or surface water bodies,
e) 50 feet between wetted area and public right of ways,
f) 100 feet between wastewater treatment units and wells,and
g) 50 feet between wastewater treatment units and property lines.
Some of the buffers specified above may not have been included in previous peunits for.this waste
treatment and disposal system. These buffers are not intended to prohibit or prevent modifications,which
are required by the Division, to improve performance of the existing treatment facility. These buffers do,
however, apply to modifications of the treatment and disposal facilities which are for the purpose of
increasing the flow that is tributary to the facility. These buffers do apply to any expansion or
modification of the spray irrigation areas and apply in instances in which the sale of property would cause
any of the buffers now complied with, for the treatment and disposal facilities, to be violated. The
applicant is advised that any modifications to the existing facilities will require a permit modification.
IL OPERATION AND MAINTENANCE REQUIRE ;NTS
1. The facilities shall be properly maintained and operated at all times.
2. Upon classification of the facility by the Water Pollution Control System y Operators Certification
Commission (WPCSOCC),the Permittee shall employ a certified wastewater tretment rLi
plant operator to
be in responsible charge(ORC)of the wastewater treatment facilities.The operator must hold a certificate
of the type and grade at least equivalent to or greater than the classification assigned to the wastewater
treatment facilities by the WPCSOCC. The Permittee must also employ a certified back-up operator of the
appropriate type and grade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the
facility must visit each Class T facility at least weekly and each Class II, III,and IV facility at least daily,
excluding weekends and holidays, and must properly manage and document daily operation and
maintenance of the facility and must comply with all other conditions of 15A NCAC 8A.0202
3. A suitable year round vegetative cover shall be maintained on all wastewater irrigation areas.Several acres
of the land added with the most recent issuance of this permit had been cleared of trees.These areas shall
have established an adequate vegetative cover over the fields and grassed waterways around the irrigation
areas prior to initial irrigation on the fields.Agronomic rates(Plant Available Nitrogen)for the cover crop
shall not be exceeded.
4. Irrigation shall not be performed daring inclement weather or when the ground is in a condition that will
cause runoff.
5. Adequate measures shall be taken to prevent wastewater runoff from the spray field.
6. The facilities shall be effectively maintained and operated as a non-discharge system to prevent the
discharge of ally wastewater resulting from the operation of this facility.

7. The application rate shall not exceed 156 inches per acre per year on Novoymes North America Farms 1,
2,and 3,and fields VF-1,VP-2,and VF 3.
8. ' The application rate shall not exceed a cumulative loading of 73.8 inches over any twelve (12) month
period on fields J-1,7-2,and J-3 and fields OF-1,OF-2,OF-3,and OF-4.
9. No type of wastewater other than that from Novoymes North America, Inc, shall be spayed onto the
irrigation area.
10. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while
normal maintenance is being performed.
11. Public access to the land application sites shall be controlled during active site use. Such controls may
include the posting of signs showing the activities being conducted at each site.
12. Several fields have rock outcrops within the spray areas. Please buffer and avoid irrigation on these rock
outcrops.
13. If animal waste is being applied to any of the spray irrigation areas adequate protection shall be taken to
insure this material is not washed off the site. Also, if animal waste is being applied to a field, the
irrigation rate shall be reduced to an appropriate level to insure that agronomic rates are not exceeded for
that field.
14. Freeboard in the aeration and storage lagoons shall not be less than one foot at any time.
11I1. MONITORING AND REPORTING REDU1REMENTS
1, Any monitoring(including groundwater,surface water.soil or plant tissue analyses)deemed necessary by
the Division to insure surface and ground water protection will be established and an acceptable sampling
reporting schedule shall be followed.
2. Adequate records shall be maintained by the Pennittee tracking the amount of wastewater disposed. These
records shall include,but arc not necessarily limited to,the following information:
a. date of irrigation,
b. volume of wastewater irrigated,
c. field irrigated,
d. length of time field is irrigated,
e. continuous weekly,monthly,and year-to-date hydraulic(inches/acre)loadings for each field,
f. weather conditions,and
g. maintenance of cover crops.
3. The effluent from the subject facilities shall be monitored by the Pennittee at the point prior to irrigation
every March,July,and November for the following parameters:

as soon as possible, but in no case more than 24 hours or on the next working day following the
occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of significant
amounts of wastes which arc abnormal in quantity or characteristic, such as the dumping of the
contents of a sludge digester;the known passage of a slug of hazardous substance through the facility;
or any other unusual circumstances.
b. Any prods unit failure, due to known or unknown reasons, that render the facility incapable of
adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,
compressors,etc.
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to
receiving waters without treatment of all or any portion of the influent to such station or facility.
d. Any time that self-monitoring information indicates that the facility has gone out of compliance with
its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter fora within five
(5) days following first knowledge of the occurrence. This report must outline the actions taken or
proposed to be taken to ensure that the problem does not recur.
IV. GROUNDWATER REOUYREMENTS
1. For purposes of reporting data, the following changes shall be made regarding the identities of the
monitoring wells:
MW-4A will be reported as MW-l;
MW-7A will be reported as MW-2;
MW-SA will be reported as MW-3;
MW-I2A will be reported as MW-12;and
MW-I6A will be reported as MW-16.
2. Existing monitor wells MW-I, MW-2, MW 3, MW-II, MW-12, MW-13, MW-14, MW-15, MW-16,
MW-17, MW I3, MW-19, MW-20, MW-21, MW-22, MW-23, MW-24, and MW-25 shall be sampled
every March,July and November for the following parameters:
Water Level pH Sulfates
Chloride Total Dissolved Solids('iDS) Ammonia Nitrogen
Manganese Total Organic Carbon(TOC) Nitrate Nitrogen

The measurement of water levels must be made prior to sampling for the remaining parameters. The depth
co water in each well shall be measured from the surveyed point on the top of the casing.
The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative
elevation of the measuring point for each monitoring well.
If TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional
sampling And analysis must be conducted to identify the individual constituents comprising this TOC
concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/1,
this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances
of this naturally occurring TOC concentration in the downgradicnt wells shall be subject to the additional
sampling and analysis as described above.
The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality
Monitoring: Compliance Report Form)by the Groundwater Section, Permits and Compliance Unit, 1636
Mail Service Center,Raleigh,N.C. 27699-1636 on or before the last working day of the month following
the sampling month.
3. Any additional groundwater quality monitoring,as deemed necessary by the Division,shall be provided.
4. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L,
Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system
constructed after December 31, 1983 is established at either(1) 250 feet from the waste disposal area, or
(2)50 feet within the property boundary,whichever is closest to the waste disposal area. An exceedance
of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate
remediation action in addition to the penalty provisions applicable under General Statute 143-
215.6A(a)(1).
5_ In accordance with I5A NCAC 2L, a REVIEW BOUN ARY is established around the disposal systems
midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance
of standards at the Review Boundary shall require remediation action on the part of the permitree.
V. INSPECTIONS
1. Adequate inspection, maintenance, and cleaning shall be provided by the Perminee to insure proper
operation of the subject facilities.
2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of
wastes to the environment,a thre,at to human health,or a nuisance. The Permittee shall keep an inspection
log or summary including at least the date and time of inspection, observations made, and any
maintenance, repairs, or corrective actions taken by.the Permittee. This log of inspections shall be
maintained bythe Permittee foraperiod of threeyears from the date of the inspection
and shall be made
available upon request to the Division or other permitting authority.
3. Any duly-authorized officer, employee, or representative of the Division may, upon presentation of
credentials,enter and inspect any property,premises or place on or related to the disposal site or facility at
any reasonable time for the purpose of determining compliance with this permit;may inspect or copy any
records that must be maintained under the terms and conditions of this permit, and may obtain samples of
groundwater, surface water, or leachate.
VI. GENERAL CONDITIONS

1. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of
this permit,the approved plans and specifications,and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the application
and other supporting data.
3. This permit is not transferable. In the event there is a desire for the facilities to change ownership,or there
is a name change of the Pernittee,a fond permit request must be submitted to the Division accompanied
by an application fee,documentation from the parties involved,and other supporting materials as may be
appropriate. The approval of this request will be considered on its merits and may or may not be
approved.
4. Failure to abide by the conditions and limitations contained in this permit may subject the Pennittee to an
enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-
215.6C.
5. The issuance of this permit does not preclude the Perm ittee front complying.with any and all statutes,
rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and
federal)which have jurisdiction.
6. The following are approved sites for disposal of treated wastewater by spray irrigation by Novoxymes
North America,Incorporated:
