5/11/2021 Overcast site visit 1.0 Hunter and I went back out to see how the clean up had gone. The
employee had done as good as job as he could, but it was
determined a clean-up company was needed. Clint had been back
out there as well and thought the same thing.
5/11/2021 Overcast phone call 1.0 Clint and I had multiple phone conversations throughout the
afternoon with Dylan discussing what was needed. Dylan wanted
to talk to all parties in order to avoid a previous incident. (A couple
months before there was a spill from grease cycle on the highway
that involved the county as well.) Eventually Dylan told us that he
had contracted with Shamrock to clean up the affected area.
However, he contracted them for non-emergency cleanup, which
meant they wouldn't be there for a couple of days. (He did not
want to pay for emergency services.) It was calling for rain that
evening, so we warned Dylan that if it rained before Shamrock
could clean up and product entered the stormwater system, then
Greasecycle would receive an NOV. He was willing to take that
chance.
5/11/2021 Overcast phone call 0.5 I talked to someone with Shamrock, and he said he (or someone)
would stop by the site early the next morning to at least put a
boom at the curb inlet.
5/12/2021 Overcast site visit 1.0 I stopped by on my way to work and saw that there was a boom in
place.
5/12/2021 Rain site visit 1.0 Hunter and I went to the site since it had started raining. Product
was entering the storm drain despite the boom that was placed
there. Photos and a video show this happening.
5/13/2021 Overcast site visit 2.0 Clint went to see the clean-up by Sharmrock and went back again
in the afternoon to see it after it was completed. Everything looked
satisfactory.
5/13/2021 Clear email 1.5 An NOV was sent to Greasecycle via email to Dylan Gehrken. I
reiterated that the NOV was because of the accident happening but
for failure to take quick action to respond to the spill.
7/12/2021 Clear email 0.5 An email was sent to Dylan requested the written response that
was past the deadline.
7/12/2021 Clear email 0.5 Written response was received.
8/13/2021 Clear email 1.0 Correction letter was sent thus closing out the NOV.

We received a phone call through our spill pager from the city's fire department about an accident at
the corner of Lindsay and Church St where about 5 quarts of gasoline leaked out of one of the cars in
the accident. Hazmat responded to the call and built a dirt berm next to the storm drain downstream to
prevent the gasoline from going inside the storm drain.

I arrived at the site and Field Ops was already there spreading sand on the road where there was still some
gasoline. Hazmat was downstream next to the first impacted storm drain, and where they had built a berm.
They added a containment boom inside the storm drain, as well as downstream at an outlet along the
stream at the corner of E Lindsay and Cumberland St. They also added another containment boom at the
stream next to the Greensboro
Curb Farmers Market.

3/21/2022 Clear site visit 1.0 I stopped by the site to check that all containment booms were
removed from the storm drains as well as the ones in the stream
downstream.
