Description of Events: On Monday, June 27. 2022, Lamberts Cable Splicing Company struck a twelve inch
rec:aim pipe while boring under the road surface At 13:03 the operator saw water flowing I was notified of
the leak at 13:12. 1 arrived at the site of the leak at 13:17. W-th the assistance of other town personal we were
able to isolate the damaged section of line resulting in the leak stopping at 13:35.
Duration of Release: Thirty-two minutes
Estimated Gallons of Release: 20,000 gallons
Did Reclaimed Water Reach Surface Waters? No, water was collected in a storm water catch basin.
Description of Water Quality at the Time of Water Release: I was unable to obtain a sample due to muddy
conditions. Grab sample of reclaim water taken resulted in the following.
Chlorine Residual-1.00 mg/I
pH-8.06
Temperature-26.9 degrees Celsius
Cause of Release: Under road utility boring, struck the reclaim line,

Corrective Actions Taken: We have reviewed our emergency response plan, we acted as soon as we were
notified of the leak, shut off flow to the area, and now awaiting the repairs to be completed. In the future we
plan to verify location and depth of all utility lines.
