Division of Water Resources' Central and Regional staff has reviewed the application package
received March 31, 2022. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on July 28, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through F, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Cover Letter:

B. Residuals Land Application Program (FORM: RLAP 06-16) Application:
Section III, Item 4 states that there is 50.62 pounds of plant available nitrogen (PAN) per dry ton
(lb PAN/DT) of residuals generated based on the January 24, 2022 residuals analysis. This totals
12,655 lb PAN annually at the permitted rate of 250 DT per year (DT/yr). Calculations of the total
PAN uptake for the permitted fields, based on the Realistic Yield Index for the soils and
management practices of these fields (including a 25% reduction of application rates on grazed
fields), suggest that there isn't enough permitted land to maintain agronomic rates at the permitted
dry tonnage with the removal of the 25.5 acres from fields NC-DV-05.
Please address Section 3, Item 6 in the application which requires a plan and schedule to resolve
any known issues that would prevent land application of the proposed residuals due to the violation
of North Carolina Administrative Code.
C. Operation and Maintenance Plan (O&M Plan):

D. Residuals Source Certification (FORM: RSC 06-16) Form:

E. Land Application Site Certification (FORM: LASC 06-16) Form:
1. The maps provided in the application are missing the delineation of the compliance and review
boundaries for the land application sites as required by 15A NCAC 02T .I 104(C)(1), and they don't
include the a note indicating the delineation of these boundaries as allowed by the June 9, 2011
memo "Map Guidance for Residuals Land Application Fields (Non -Dedicated)". Please provide
updated maps that include this delineation or a note of the delineation.
F. Landowner Agreement (FORM: LOA 06-16) Form:
