Form 2F Application for Permit to Discharge Storm Water Associated with Industrial Activity - Who Must File Form 2F Form 2F must completed by operators of facilities which discharge storm water associated with Industrial activity or by operators of storm water discharges that EPA is evaluating for designation of a significant contributor of pollutants to waters of the United States, or as contributing to a violation of water quality standard Operators of discharges which are composed entirely of storm water must complete Form 2F (EPA Form 3510 -
