Dear Mr. Stowe:
Enclosed is a copy of the Compliance Sampling Inspection Report,for the inspection
conducted at the,subject facility on June 7, 2010 by Ms. Donna Hood and Mr. Wes Bell of the
Mooresvtle'Regional Office and,accompanied by Ms Autumn Romanski of the Raleigh
Regional Office. Please advise the facility's Operator -in -Responsible Charge of our findings by
forwarding a copy of the enclosed report. The results of the analyses collected by DWQ Staff
will be se -fit to you under separate cover.
'-The facility has several stormwater outfalls that discharge'into waters of the State
Although .he facility operates under an internal stormwater management program, potential
exposures to the,stormwater runoff as a result of the facility',s operations exist. Following
discussions with the Division's Stormwater Permitting Unit, the stormwater discharges will need
to be covered under an individual stormwater permit. A completed permit application and
supporting documentation (including fee) should be, mailed to the attention of Bradley Bennett,
Supervisor, DWQ/Stormwater Permitting Unit, 1617 Mail Service Center, Raleigh, N.C. 27699-
1617. The permit application and supporting information can be found at the following website

application and supporting documentation (including fee), should be submitted, to the Division
within 30 days'following receipt of this report.

stormwater exposures (from raw materials, by-products, metal storage dumpsters, etc ) as a result of the
facility's operations were observed The facility staff operate under a l Internal stormwater management

FACILITY SITE REVIEW
The Division Issued an ATC on 4/27/10 for'the Installation of vertical flow wetland ,(VFW) cells containing
spent mushroom, compost for, the removal of selenium These VFW wetland cells will replace the existing
wetland cells 1A„ 1 B, and 1 C These wetland cells (1A, 1 B, 1 C) have been taken off line and the vegetation
from one,of,these wetland cells has already been removed The facility staff Indicated that the -vegetation
would be composted and, placed back Into an on-line wetland cell(s)
SLUDGE DISPOSAL
Fly ash, clarifier sludge, spec and off spec gypsum, etc are disposed Into the permitted landfills located
on-site The spec gypsum Is also reused'In the National Gypsum Company's manufacturing process

to determine if an,additional classification (Grade I Biological) is warranted The TACU
