Division of Water Resources' Central and Regional staff has reviewed the application package
received January 21, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 2,
2022.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through M, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Application:
1. Section I, Item 3 — Pursuant to 15A NCAC 02T .0106(b)(4), applications for municipalities shall
be signed by either an executive officer (i.e., City Manager), elected official in the highest level of
elected office (i.e., Mayor), or an authorized employee. Review of our database indicates that Steve
Craver is not authorized to sign this renewal application on behalf of the City of Lexington.
Accordingly, please either resubmit an application signed by either the City Manager or Mayor, or
provide a signature authority delegation letter pursuant to 15A NCAC 02T .0I06(c). The signature
authority delegation letter shall be signed by either the City Manager or Mayor, and delegates Steve
Craver to sign on behalf of the City of Lexington for the Permit No. WQ0016165 renewal

Section V — Review of our database indicates there are two invalid back-up ORCs for the subject
permit. They are Elmer J. Camp (SI/991942) and Roger A. Spach (SI/22808). Please contact the
Technical Services Branch's Operator Certification Unit to either have these operators removed
from the subject permit, or to have their licenses reactivated.
