On April 30, 2014, the Division of Water Resources (Division) received your application dated April
24, 2014 requesting a 401 Water Quality Certification from the Division for your project. Revised
information was received on May 20, 2014. The Division has determined that your application is
incomplete and cannot be processed because additional information is needed to complete the
review of diffuse flow for this project. The application is on -hold until the following information is
received:
1. A detail view of level spreader F, splitter box, and bypass channel must be provided as well
as the drainage area collection structure on level spreader detail sheet (SW -LS1).
2. A supplement form must be provided for level spreader F.
3. If the slope in the riparian buffer exceeds 5% or the 1 in /hr storm from the drainage area
exceeds 2 cfs, then an LS may not be used adjacent to the buffer. Instead of diffuse flow, a
BMP that removes at least 30% TN and 30 %TP must be provided. This comment applies to
level spreaders D and E.
4. Supplement forms must be fully completed including the Bypass System Design section.
Peak velocity in the channel for the 10 -yr storm must be provided as well as calculations
demonstrating non - erosive flow through the buffer.

5. Calculations for level spreaders A, B, and C were not provided. Please provide calculations
supporting the 10 -year peak discharge rate for each BMP that discharges to a LS -VFS and
bypass invert elevations.
6. Please provide bypass outlet structure design details for each BMP with discharge to LS -VFS
(dimensions, invert elevations, type, etc) on the level spreader detail sheet (SW -LS1).
Pursuant to Title 15A NCAC 02H .0502(c) and Title 15A NCAC 02B .0233 (8), the applicant shall
provide all of the above requested information for the proper consideration of the application. If
all of the requested information is not received in writing within 30 calendar days of receipt of this
letter, the Division will be unable to approve the application and it will be returned. The return of
this project will necessitate reapplication to the Division for approval, including a complete
application package and the appropriate fee.
Please respond in writing within 30 calendar days of receipt of this letter by sending four copies of
all of the above requested information to the Wetlands, Buffers, Stormwater — Compliance and
Permitting (Webscape) Unit, 1650 Mail Service Center, Raleigh, NC 27699 -1650.
Please be aware that you have no authorization under the Water Quality Certification Rules for
this activity.
