Reason for Visit: Routine
Inspection Summary:
This is an Annual Report review. A site visit was not conducted as part of this review.
SAR and PAN values with populated calculations were presented for all four quarters. These values reported are within the
limits of the permit.
The Soil Analysis for the field that received residual material indicates no lime amendments were required for the fields that
received residual material.
The C.O.C.for 1/4/2021 was dated 1/4/2020. The laboratory receiving sheet had the correct date of 1/5/2021. Also, there
was no preservation method noted on the 1/4/2021 C.O.C. Please be sure to properly date and notate the preservation
method on all Chain of Custody documents. The C.O.C. is a legal documents and needs to be completed correctly.
