On February 13, 2012, Mandy Hall of the Raleigh Regional Office visited your single-family residence
(SFR) wastewater treatment system to evaluate compliance with the above permit to discharge
wastewater. The assistance of your son, Mr. Morgan Haskell, was greatly appreciated. The checked
box below show what condition(s) was noted at your facility:
® In compliance. You are reminded to regularly maintain the chlorine disinfection system with
wastewater -rated chlorine tablets and have the effluent sampled once per year. Even if your
facility does not discharge have the septic tank pumped out every 3 to 5 years. If your system
does discharge, have the effluent sampled once per year.

Licensed plumbers should be used to make plumbing changes within your home.

Met with Morgan Haskell, son of owner and current resident in the home.
Treatment units are present and functional but at the time of the inspection, did not find the system to be
discharging.
Mr. Haskell explained the occupancy of the home is sporadic. There were chlorine tablets in the chlorinator.
