Location description: The Wellesley Phase 5 Subdivision is located on the south side of Cleveland Road (S.R. 1010), between the
intersecting streets Watkins and Matthews roads, in the community of Cleveland, Town of Clayton, Johnston County, NC.
Description of projects area and activity: This verification authorizes the use of Nationwide Permit (NWP) 29 to construct
impacts to waters associated with the proposed Wellesley Phase 5 Subdivision at three locations. Impact Area 1 would
construct a road crossing and require the permanent impact to 39 linear feet Of) (0.0098 ac), of intermittent Stream A (Impact
S1), an additional 71f (0.0015 ac) of temporary impact to Stream A (Impact S1) to accommodate a stream pump around,
0.0039 acre of permanent impact to Wetland B (Impact W1), 0.001 acre of temporary impact to Wetland B (Impact W2), 147
square feet (sf) (0.0034 ac) of permanent impact to Pond B (Impact 01), and temporary impact to 23 sf (0.0005) of Pond B
(Impact 02) to accommodate a pump around. Impact Area 2 would construct a Stormwater BMP outfall riprap dissipator
and require the permanent impact to 62 sf (0.0014 ac) of Pond B (Impact 03), and the temporary impact to 59 sf (0.0014 ac) of
Pond B (Impact 04). Impact Area 3 would repair and replace an existing outlet structure connecting Ponds B and A, and
require the permanent impact to 84 sf (0.0019 ac) of Pond B (Impact 05), and the temporary impact to 2882 sf (0.0662 ac) of
Ponds B and A (Impact 06).

Your work is authorized by the above referenced permit provided it is accomplished in strict accordance with the attached
Conditions, your application signed and dated 1/31/2022, and the enclosed plans Wellesley Phase 5 (nine sheets) undated. Any
violation of the attached conditions or deviation from your submitted plans may subject the permittee to a stop work order, a
restoration order, a Class I administrative penalty, and/or appropriate legal action.
This verification will remain valid until the expiration date identified below unless the nationwide and/or regional general permit
authorization is modified, suspended or revoked. If, prior to the expiration date identified below, the nationwide and/or regional general
permit authorization is reissued and/or modified, this verification will remain valid until the expiration date identified below, provided
it complies with all requirements of the modified nationwide permit. If the nationwide and/or regional general permit authorization
expires or is suspended, revoked, or is modified, such that the activity would no longer comply with the terms and conditions of the
nationwide permit, activities which have commenced (i.e., are under construction) or are under contract to commence in reliance upon
the nationwide and/or regional general permit, will remain authorized provided the activity is completed within twelve months of the
date of the nationwide and/or regional general permit's expiration, modification or revocation, unless discretionary authority has been
exercised on a case -by -case basis to modify, suspend or revoke the authorization.
Activities subject to Section 404 (as indicated above) may also require an individual Section 401 Water Quality Certification. You
should contact the NC Division of Water Resources (telephone 919-807-6300) to determine Section 401 requirements.
For activities occurring within the twenty coastal counties subject to regulation under the Coastal Area Management Act (LAMA), prior
to beginning work you must contact the N.C. Division of Coastal Management Morehead City, NC, at (252) 808-2808.
This Department of the Army verification does not relieve the permittee of the responsibility to obtain any other required Federal, State

Maintenance Inspect rock pipe inlet protection at least weekly and efier each significant (IA
Inch or greater) rainfall event and repair immediately. Remove sediment and
restore the sediment storage area to its original dimensions when the sediment
has accumulated to one-half the design depth of the trap. Place the sediment
that is removed in the designated disposal area and reptace the contaminated
pan of the gravel facing.
Check the structure for damage. Any riptap displaced from the stone horseshoe
must be replaced immediately.
After all the sediment -producing areas have been permanently stabilized,
remove he structure and all the unstable sediment. Smooth the area to
Mend Acilh the adjoining areas and provide permanent ground cover (Surf ce
Stshit7_ntim).
