follous:
1. Inspect the septic tank annually, and pump out solids as needed.
2. Inspect and clean the septic tank effluent filter annually. (J/app/lc¢®/e)
3. Inspect the tablet chlorinator weekly. Add wastewater grade chlorine tablets (e.g., calcium hypochlorite) as needed.

4. Inspect the ultraviolet disinfection unit weekly. Clean or replace the lamps and quartz sleeves as needed. (or

5. Inspect all storage tanks, pumps, and alarms monthly. Remove the floating scum layer in all pump/storage tanks

6. Inspect the spray irrigation system monthly to verify: proper operation of the spray heads; that there are no leaks;
that vegetative growth does not obstruct the spray heads; that the irrigated wastewater is not ponding in or running

7. Inspect the drip irrigation system monthly to verify: proper operation of the drip lines; that there are no leaks; that

It has come to the Division of Water Resources' attention that the subject singlerfemily residence
wastewater irrigation facility has changed ownership. The Union County Register of Deeds indicates that
Jeffrey E. Brink and Ms. Tracy 8. Legeza purchased the subject property q'arcel No. 05-162009) from
John and Synda Bass on Febnrary 21, 2020 (Deed Book: 7590 / Page: 373-375).
The wastewater treatment and irrigation system associated with this property are covered under an
individual State-issued non-dischange permit (Permit No. WQ0032548). Accordingly, please complete and
submit the attached Non-Dischange Permit Ownership/Name Change application Q7orm: NDPONC) and
the Single-Family Residence Wastewater Irrigation System Operation and Maintenance Agreement oTom:
SFRWWIS O&M). These forms have been prefilled for your convenience.
Once the application is received, Mooresville Regional Office staff will conduct a site visit and
provide an overview of the treatment and disposal system. Upon completion of the site visit and review of
the application, a new non-discharge permit will be issued for a length of up to eight years. There is a $60
annual fee associated with this permit, which is not due at this time.
Please complete and sul)nit the attached forms within 30 davs of receivivinE this notification.
Failiire to provide these forms chall result in a Notice of Deficiency for operating the subject facilities
without a valid permit, which is a violation of G.S. 143-215.1, and may subject you to a Notice or
Violation and appropriate enforcement actions pursuant to G.S. 143-215.6A-6C. Civil penalties or

Dear hfr. and b4rs. Bass:
In accordance with your pemit change of ownership request received April 3, 2017, we are
forwarding herewith Permit No. WQ0032548 dated June 29, 2017, to John R. and Sydna Bass for the
continued operation of the subject single-family residence wastewater treatment and irrigation facilities.
Please note that the Permittee has been changed from Charles and Beverly MCLanb to John R. and
Sydna Bass.
This permit shall be effective from the date of issuance until May 31, 2022, shall void Permit No.
WQ0032548 issued April 8, 2008, and shall be subject to the conditions and limitations as specified therein.
An extension sball be requested at least Six months prior to the expiration of this permit. Upon receipt
of the request, the Commission will review the adequacy of the facility described therein and, if warranted,
will extend the permit for such period of time and under such conditions and limitations as it may deem
appropriate.
Please pay particular attention to the operation and maintenance requirements listed in Section Ill
for they may differ from the previous permit issuance. Failure to establish an adequate system for collecting
and maintaining the required operational information shall result in future compliance problems. Also note
that Condition VI.6. requires the Permittee to retain a set of Division approved plans and specifications for
the life of the facilities pemitted herein.
If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 1508 of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service
Center, RAleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and binding.
