the FRO form, NOI Certification form, and Tab E (certification tab).

D1. Ensure this date matches the approval/issue date of your E&SC documentation (grading permit/letter of approval).

Form (Tab D9). If signing as authorized responsible person, that person must also sign the NOI certification form.
