received February 3, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on June 3,
2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through G, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

F. Land Application Site Certification (FORM: LASC 06-16) Form:
Item B on the Instruction for FORM: LASC 06-16 requires that the Permittee shall provide a soils
and agronomic evaluation in accordance with the Soil Scientist Evaluation Policy for all new sites.
The application included a report from the licensed soil scientist (LSS) as well as soil analyses for
the pollutants listed in Rule 15A NCAC 02T .1105(b). However, it did not include a fertility
analysis that includes the remaining parameters listed in 02T .1104(c)(2)(B) and item 4.c.ii.1. on
page 15 of the Soil Scientist Evaluation Policy. This fertility analysis shall be submitted for each
new field included in this application.
2. Item B on the Instruction for FORM: LASC 06-16 requires that an updated accounting of the land
application site's cumulative pollutant loading rates (CPLRs) be provided for renewed land
application sites. Some of the CPLRs included in the application are from prior to 2021, including
NC-AM-002, AM-006-02 and 08, AM-007, AM-008, and AM-035-02. Please provide the most
updated accounting for these sites.
According to the Winston-Salem Regional Office (WSRO), there are offsite houses under
construction within close proximity of the application area for the following fields:
a. NC-AM-002-02;
b. NC-AM-015-04; and
c. NC-CH-039-02
These places of public assembly under separate ownership shall be shown on the resubmitted site
map for this field with the correct setbacks, and any acreage change shall be included in an updated
LASC form.

4. There is a house owned by the signatory of the landowner agreement of field NC-AM-019 that is
not shown on the site map. The address of this house is 2542 Barnett Road. This house shall be
shown on an updated site map with any associated appurtenances (such as a water supply well) and
setbacks included.
5. The two site maps provided for NC-AM-016 did not show the location of fields 01, 02A, 02B, 04A,
and 4B. These maps shall be provided for review.
6. The map for field NC-AM-100 depicts parcel 9727455619 owned by Lawrence and Dorothy
Herron and parcel 9727351083 owned by Michael and Derly Carolina Stealey as a part of the
project boundary. These parcels shall be removed from the site boundary in an updated map with
the appropriate parcel boundary setback. Any acreage change shall be included in an updated LASC
form.
7. Fields NC-AM-0 1 6-02B and 4A, NC-AM-110-01, NC-AM-126-02, and NC-AM-126-03 were
modified to include the areas that were not formerly included due to the property line setback.
However, these areas shall still be certified to be suitable for land application. An abbreviated soils
evaluation in accordance with Section B of the Instruction for FORM: LASC 06-16 shall be
submitted to verify that these sections of each field is consistent with the soil series currently listed
in the permit, and this shall be signed and sealed by a Licensed Soil Scientist (LSS).
Fields NC-CS-036-08 and 09 were modified to include the areas that were not formerly included
due to surface water setbacks present in each fields. WSRO mentioned that these features were
removed after remediation efforts. However, these areas shall still be certified to be suitable for
land application. An abbreviated soils evaluation in accordance with Section B of the Instruction
for FORM: LASC 06-16 shall be submitted to verify that these sections of each field is consistent
with the soil series currently listed in the permit, and this shall be signed and sealed by a Licensed
Soil Scientist (LSS).
9. The map for field NC-CH-251 shows an onsite house on parcel 62637 owned by Donna and Richard
Byrd. The setback to this house was waived in the previous permit, but was not renewed in this
permit issuance. The setbacks for a place of public assembly under separate ownership shall be
shown on the resubmitted site map for this field, and any acreage change shall be included in an
updated LASC form. Alternatively, setback waivers may be executed in accordance with 15A
NCAC 02T .1108(e) to reduce a setback. Setback waivers shall be written, notarized, signed by all
parties involved, and recorded with the Chatham County Register of Deeds.
G. Landowner Agreement (FORM: LOA 06-16) Form:
1. The LOAs provided for field NC-CS-113-1,2 signed by Elizabeth Simmons Kirby and NC-CS-
113-6A,6B signed by Jimmy D Allred do not appear to be notarized. Please provide copies of these
LOAs that are notarized in accordance with 15A NCAC 02T .1104(c)(4).
2. The provided LOA for field NC-AM-055-05 was signed by Joseph Rice, but the deeded owner on
parcel 8797312076 is Sam Ira Kiser Jr. and Deborah Kiser. Please provide an updated LOA signed
by one of these deeded owners. Otherwise, an updated LASC shall be provided stating that this
field is being deleted.
3. The provided LOA for field NC-AM-126-09A was signed by Thomas Alvah Ashley, but the deeded
owner on parcel 8767299180 is Ralph Carter Ashley. Please provide an updated LOA signed by
this deeded owner. Otherwise, an updated LASC shall be provided stating that this field is being
deleted.
