Dear Mr. Sink:
Division of Water Resources staff Caitlin Caudle completed a review of the 2021
Residuals Annual Report. This review reflected non-compliance with the subject permit.
The following violation was noted:
1. Based on the information provided in the 2021 Annual Report, the total dry tonnage
applied for the source Davidson Water WTP was over the permitted limit. The dry
tonnage applied based on the annual report was 772.36 tons. The permitted
amount for this facility is 566 dry tons per year. If application activities are
expected to be greater than 566 dry tons per year moving forward, please
submit a permit modification application to increase the permitted dry tons
for this facility.
Please notify the Winston Salem Regional Office prior to your next land application event
that is planned to occur to allow for a routine compliance inspection.

Inspection Summary:
Division of Water Resources staff Caitlin Caudle completed a review of the 2021 Residuals Annual Report. This review
reflected non-compliance with the subject permit. The following violation was noted:
1. Based on the information provided in the 2021 Annual Report, the total dry tonnage applied for the source Davidson

Other observations:
• TCLP was not completed in the 2021 calendar year. TCLP is required once per permit cycle.
• Residuals were applied to DV-08-07, 09, and 10 in the WQ0010583 Davie County RLAP permit. Application events
occurred in May, September, and December.
• Quarterly or 4x year sampling was required. Sampling was completed on 5/3 and 10/19. On 10/19 samples were
collected for liquid residuals and cake residuals, and one sample was labeled as clean out.
• All pH values were less than 6.
• PAN for DV08-07 and 09 were reported as 198 Ib/ac. According to NCSU RYE database the PAN for these fields is 188
Ib/ac.
