Hello Ms. Daniels,
On April 4, 2022, the Division of Water Resources (Division) received your application requesting a 401 Water Quality
Certification from the Division for the subject project. The Division has determined that your application is incomplete
and cannot be processed. The application is on‐hold until all of the following information is received:
1. Please provide a site plan showing the property boundaries, structures (buildings, docks, impervious surfaces,
etc.), location of the shoreline, location of the impacts to the shoreline and/or lake bottom (excavation,
dredging, rip rap, retaining walls, etc.)[15A NCAC 02H .0502(a)(9)]
2. Please provide an elevation plan showing the approximate location of rip rap or fill to be placed above and
below the Full Pond/ Normal Water Level elevation. The template located on pages 4‐5 of the help document
linked below may be used to assist with this requirement. [15A NCAC 02H .0502(a)(9)]

3. Within your application, you state that work will be performed to shore up the remaining wall. Per Condition 11
GC 4502, all necessary measures shall be taken to prevent direct contact between uncured concrete and waters
of the state. Please provide information and details regarding materials used as well as their placement method
along the shoreline. [15A NCAC 02H .0502(a)(9) and .0506(b)(3)]
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please respond in writing within 30 calendar days. If all of the requested
information is not received within 30 calendar days of receipt of this letter, the Division will be unable to approve the
application and it will be denied as incomplete. The denial of this project will necessitate reapplication to the Division
for approval, including a complete application package and the appropriate fee.
Please be aware that you have no authorization under the Section 401 of the Clean Water Act for this activity and any
work done within waters of the state may be a violation of North Carolina General Statutes and Administrative Code.
