received November 9, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on February
3, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through D, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.

revised and/or additional documentation shall be signed, sealed and dated (where needed), with one

A. Field NC-RC-012-02:
1. This field needs to be split into two separate fields. The portion next to Truman Road is mapped as
Rhodhiss, which has a PAN rate of 167 lb/ac for fescue hay. The portion in the woods is mapped
as Clifford, which has a PAN rate of 209 lb/ac for fescue hay. This field can easily be split into two
management areas. Suggested field separations see attached map. Please update and resubmit the
Land Application Site Certification (LASC) and the site map.
B. Field NC-RC-015-01:
1. This field also needs to be split into several fields due to differences in soil types and the size of
the field. There are several individual management areas separated by access roads, Separating the
field will also provide an easier way to track residuals application. The predominate soil series in
this field are Clifford (209 lb/ac for fescue hay), Fairview (154 lb/ac for fescue hay), and Nathalie
(178 lb/ac for fescue hay). Suggested field separations see attached map. Please update and
resubmit the LASC and the site map.
C. Field NC-RC-126-06:
1. The correct soil series is Rhodhiss for Field NC-RC-126-06, please update and resubmit the LASC
form.
D. Recommendations: (Response not required)
1. The permit issued June 4, 2020 did not include site maps for the following fields: NC-RC-002-01,
NC-RC-010-01, NC-RC-011-02, NC-RC-012-01, NC-RC-019-01, NC-RC-019-02, NC-RC-019-
03, NC-RC-019-04, NC-RC-019-05, NC-RC-118-01, NC-RC-118-02, NC-RC-125-05, NC-RC-
125-06, NC-RC-126-03, NC-RC-126-04, NC-RC-128-01, NC-RC-128-02, NC-RC-128-03, NC-
RC-130-01. These maps are not required for this modification. However, it is the Permittee's
responsibility to ensure that a complete permit is maintained.
