Division of Water Resources’ Central and Regional staff has reviewed the application package
received January 28, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on May 18,
2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through E, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A. Overall Submittal Package:
1. Within the application package an enclosure titled “Updated facility description” was included. This
description includes several modifications that have been made at Holly Ridge WWTF, notably that
the number and type of aerators in the treatment lagoon were modified, the configuration of the
treatment lagoon was modified with the addition of a fourth aeration cell and the reduction of the
settling cell from 0.49 million gallons (MG) to 0.3 MG, and the type of disinfection has been changed
from ultraviolet (UV) disinfection to chlorine disinfection. These changes are all minor modifications;
however, our office has no record of these modifications in our files. Modifications to the treatment
system require a permit modification application be submitted to the Non-Discharge Branch. Please
submit application form WWIS 06-16, signed and sealed engineering documents pursuant to 15A
NCAC 02T .0504. Please provide information regarding the modified aeration system, lagoon
configuration, and disinfection system, as well as any piping or other appurtenance information that
was required for its installation. Our understanding is that this equipment has already been installed,
as such, please also provide the date it was installed, an executed engineering certification, and the
updated Operation and Maintenance (O&M) plan.
