Dear Mr. Weatherly:
In accordance with your application received April 13, 2022, we are forwarding
herewith Well Construction Permit No. WS0701169, dated April 14, 2022, issued to
Currituck County for the construction of a public water supply well (R04) on property
owned by Currituck County on Maple Road, Maple, Currituck County, North Carolina.
This Permit will be effective from the date of its issuance until April 14, 2023,
and shall be subject to the conditions and limitations as specified therein. Please note that
Condition 1 of Well Construction Permit No. WS0701169 requires that prior to the start
of any well drilling and/or construction activities, the well driller's name, contact
information, and North Carolina certification information shall be provided to the
Washington Regional Office of the Water Quality Regional Operations Section.
If any parts, requirements, or limitations contained in this Permit are unacceptable
to you, you have the right to an adjudicatory hearing before a hearing officer upon written
demand to the Director within 30 days following receipt of this Permit, identifying the
specified issues to be contended. Unless such demand is made, this Permit shall be final
and binding.

Groundwater information collected in the coastal plain counties indicates
declining groundwater levels in the Tertiary Aquifer System. In an effort to protect and
conserve the groundwater resources, the Division encourages water users to develop and
implement a water conservation and management plan
A Well Construction Record (GW-1) must be filled out by the driller and
submitted to Environment & Natural Resources, Attn: Information Management, 1617
Mail Service Center, Raleigh, NC 27699-1636, within 30 days upon completion of the
well construction.

PERMIT FOR THE CONSTRUCTION OF A WATER SUPPLY WELL OR WELL SYSTEM
(Wells or Well Systems with a Design Capacity of 100,000 Gallons Per Day or Greater)
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes,
and other applicable Laws, Rules and Regulations:
PERMISSION IS HEREBY GRANTED TO
Currituck County
FOR THE CONSTRUCTION OF a Public Water Supply Well in the Tertiary Aquifer
System in Maple, Currituck County, North Carolina on property owned by Currituck County
on Maple Road, Maple, Currituck County, North Carolina. This Permit is issued in accordance
with the application received on April 13, 2022, in conformity with specifications and
supporting data, all of which are filed with the Department of Environmental Quality and are
considered integral parts of this Permit.
This Permit is for well construction only, and does not waive any provision or requirement
or any other applicable law or regulation.
Construction of any well under this Permit shall be in strict compliance with the North
Carolina Well Construction Regulations and Standards (15A NCAC 02C .0100), and other
State and Local Laws and regulations pertaining to water supply well construction.
If any requirements or limitations specified in this Permit are unacceptable, you have a
right to an adjudicatory hearing upon written request within 30 days of receipt of this Permit.
The request must be in the form of a written petition conforming to Chapter 150B of the North
Carolina General Statutes and filed with the Office of Administrative Hearings, 6714 Mail
Service Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this
Permit is final and binding.
This Permit will be effective for one year from the date of its issuance and shall be subject
to other specified conditions, limitations, or exceptions as follows:

1. Prior to the commencement of any well drilling and/or construction activities, the well
driller's name, contact information, and North Carolina certification information shall
be provided to Randy Sipe, Water Quality Regional Operations Section, 943
Washington Square Mall, Washington, North Carolina 27889, telephone (252) 948-
3849, email randy.sipe@ncdenr.gov.
2. The wells shall have a Well Contractor Identification Plate and a Pump Installation
Information Plate in accordance with 15A NCAC 02C .0107(j)(2) and (3).
3. Well construction records (GW-1) for each well shall be submitted to the Division of
Water Resources' Information Processing Unit within 30 days of the well completion.
4. When the wells are discontinued or abandoned, they shall be abandoned in accordance
with 15A NCAC 02C .0113 and a well abandonment record (GW-30) shall be
submitted to the Division of Water Resources' Information Processing Unit within 30
days of the well abandonment.
