Division of Water Resources staff person Caitlin Caudle completed a review of the 2021
Residuals Annual Report. This review mostly reflected compliance with the subject permit.
The following item of concern was noted:
1. The 2021 Residuals Annual did not contain a Class A Annual Distribution & Marketing
Certification and Summary Form (DMSDF). This was also noted in correspondence
dated April 5, 2021. Please submit a properly completed DMSDF form in all future
annual reports for the subject Class A permit. If necessary, please also include
supplemental forms (DMSDF-Supp) to provide information that will not fit on form
DMSDF.
Please notify the Winston Salem Regional Office prior to your next land application event that
is planned to occur within the Winston Salem Region to allow for a routine compliance
inspection. Please refer to the enclosed inspection report for additional comments and

No application activities occurred in the 2021 calendar year. Application activities are planned for the Town of Forest City and
