Dear Mr. Wescott:
Please find enclosed a modified permit drawing, summary impact tables, and a work plan for the above referenced
project. The modification involves the installation of an impervious dike in Deep Creek from the bank around
the interior bent of the existing bridge. The original plan for removing the bent was by use of a crane. However,
due to the deteriorated condition of the bent, NCDOT and the project supervisor for S. T. Wooten Corporation
have concerns that removal of the bent by the crane may negatively impact water quality of Deep Creek frpm
large pieces of concrete falling into the stream. By installing the impervious dike, the area azound the bent can
be dewatered thereby minimizing any negative water quality impacts to Deep Creek. NCDOT is requesting an
additional 2100 squaze feet of temporary stream impact for installation of the cofferdams and the removal of the
bent. No additional buffer zone impacts are required. The cofferdams will be removed immediately upon
completion of the work.
Best Management Practices for the Protection of Surface Waters and for Bridge Demolition and Removal will be
strictly adhered to during the removal of the existing bridge.

WBS 40135.1.1 8-4934; Bridge 85 over Deep Creek on US .Hwy 258
S. T. Wooten Corporation is requesting a permit modification for the above referenced
bridge structure, located on Hwy 258 North of Princeville, NC. This Permit Modification
request is to allow S T Wooten Corporation to install an impervious dike in Deep Creek,
dewater the area within the impervious dike, and remove the existing interior bent from
the creek.
We are still going to try our option # 1 of removing the interior bent with the crane.
However, based on the condition of the existing structure we have concerns it is not
going to work. Therefore, we are requesting this permit modif cation, to have option # 2,
which is taking the following steps in removing the existing interior bent.
1. STW will install an impervious dike from the bank around the interior bent and
back to the bank as shown on the attached drawing.
2. We will dewater the vcrork area into an erosion control silt bag.
3. Then we will access the work area from a platform supported by the existing bank
and existing crutch bent piles.
4. W'e will remove the interior bent by pulling the bent over into the work area, and
remove the pieces of concrete.
S. Then we will break the footing apart and remove all the concrete from the work
area.
6. STW will remove the impervious dike and restore the creek's flow.
7. Then continue with the construction of the new structure.
