This template mirrors the approved Stormwater Management Plan and includes: A master list of all required questions, and A sheet of specific questions for each reporting year. Please note that under the new permit, annual reports are due no later than February 28 of each year and must use this streamlined template. The annual reporting period is January 1 – December 31 of each year. Please submit only the applicable sheet/information each year. Each annual self- assessment submittal will be reviewed to ensure that the MS4 is on pace to achieve compliance. The attached file is unlocked and can be filtered. However, cell formatting may be lost if the document is sorted. It is recommended that you create and store a backup of this file.
