Dear Mr. Holcombe:
On April 5, 2022, Division of Water Resources staff Caitlin Caudle, Tricia Lowery, and Lon Snider
conducted a scheduled compliance inspection of NPDES Permit NC0005566 and Non -Discharge
Permit WQ0000461. Chad Witherspoon, James Boles, and Mike Jolly of Louisiana-Pacific
Corporation were present. Ms. Caudle completed the non -discharge inspection with the
assistance of James Boles, land application ORC. A review of available records, current residuals
management procedures, historic land application fields, and monitoring wells was completed.
This review reflected compliance with the subject permit.

Inspection Summary:
On April 5, 2022, Division of Water Resources staff Caitlin Caudle, Tricia Lowery, and Lon Snider conducted a scheduled
compliance inspection of NPDES Permit NC0005566 and Non -Discharge Permit WQ0000461. Chad Witherspoon, James
Boles, and Mike Jolly of Louisiana Pacific Corporation were present. Ms. Caudle completed the non -discharge inspection. A
review of available records, current residuals management procedures, historic land application fields, and monitoring wells
was completed. This review reflected compliance with the subject permit.
Currently, all residuals are being hauled to the landfill. Washdown from the plant enters the EQ basin, where it is then
transferred to a smaller tank where coagulation, flocculation, or other treatment occurs. Effluent is then transferred to filter
bags where the solids are filtered out. Filter bags are emptied onto a concrete pad and hauled to the landfill. Filtered effluent
returns to the industrial system and is pumped and hauled. Effluent also undergoes solidification where wood dust is mixed
with effluent on a bermed concrete pad. The resulting residuals are hauled to the landfill.
Records were available for all material taken to the landfill. Two to three loads a day are taken to the landfill.
Other observations:
• All industrial effluent is being pumped and hauled.
• Typically, <3,000 GPD are produced with average values being—1,200-1,400 GPD.
• The pump and haul permit does no require reporting to the Winston Salem Regional Office.
• All monitoring wells were in good condition.
• The field between the plant and the land application fields is managed by AJS Mathis, LLC.
• Data is being collected concerning the evaporation of liquid from the aerated lagoons. One lagoon has aeration with
about 20 aerators, two were in use at the time of inspection.
