In addition, the flow values were not being rounded properly. The flow values for all applicable months (December 2021 and January 2022 at a minimum) will be revised and resubmitted. Discharge monitoring reports (eDMRs) were reviewed for the period February 2021 through January 2022. The following effluent limit violations were reported during the noted review period: - Daily maximum total residual chlorine (TRC) - September 2021, October 2021 and November 2021, - Daily maximum fecal coliform - September 2021, October 2021, December 2021 and January 2022; - Daily minimum dissolved oxygen (DO) — September 2021 and November 2021; - Daily maximum total suspended solids (TSS) — October 2021, December 2021 and January 2022; - Monthly average fecal coliform — December 2021 and January 2022; - Monthly average TSS — October 2021 and December 2021. violation was reported in February 2021 The facility failed to monitor effluent BOD, TSS, fecal coliform, flow, ammonia, DO and pH during two weeks in September 2021 and effluent temperature was not monitored for one of those weeks. An effluent DO was also not monitored during one week in November 2021. The September 2021, October 2021 and November 2021 eDMRs were submitted late. All of the above -noted violations (limit, monitoring, late reporting) will be addressed by the Division through the issuances of either Notice of Violations (NOV) or NOV/civil penalty assessments. The ORC must ensure to check the Noncompliant box on the eDMR if any violations (limit and/or monitoring) occurred during the month. LABORATORY SECTION cont'd: The DO meter did not appear to be calibrated properly. The DO meter must be calibrated according to the manufacturer's instructions and documented (Reference: NC Wastewater/Groundwater Laboratory Certification approved procedure for the analysis/calibration of DO). The ORC must ensure to perform (and document) a calibration check standard verification (7.0 SU buffer) following the initial calibration of the meter with the three buffers/standards. The calibration check standard must fall within +/- 0.1 SU of the standard (Reference: NC Wastewater/Groundwater Laboratory Certification approved procedure for the analysis/calibration of pH). If effluent field analyses are performed at multiple sites in a single day, then a post calibration check must be performed (and documented) on the DO, pH and TRC meters following the last site visit that field analyses have been performed (Reference: NC Wastewater/Groundwater Laboratory Certification approved procedure for the analysis/calibration of DO, pH and TRC). Note: The ORC has been previously emailed a link to the Division's Laboratory Certification Unit webpage to access the approved procedures for the analysis/calibration of DO, pH, temperature and TRC). The ORC and staff must ensure that the calibration logs including the following: model and serial number of the meter, proper units of measurement, times of calibration and the analyst performing the calibration. The ORC is in the process of obtaining certification for TRC from the Division's Laboratory Certification Unit; however, DO and temperature should also be included in the certification process. The ORC and staff should review and become familiar with the treatment facility's operation and maintenance manual and be able to make adjustments if needed. In addition, the ORC and staff should initiate (and document) process control testing to ensure proper facility operation. Please be advised that the NPDES Permit [Part II, Section C(2) Operation & Maintenance of Pollution Controls] requires the permittee to properly operate and maintain the wastewater treatment facility at all times. These measurements will assist with the operation of the biological treatment process and the maintenance of the solids inventory. In addition, solids were observed entering the chlorination system once the decant pump was activated. Sludge blanket measurements should be periodically performed and documented on the clarifier. It is also recommended that the location (depth) of the decant pump system be determined to ensure the sludge blanket level is below the pump inlet structure.
