the correct number.
A15c & d. From the map, it also looks like this project area will drain to Big Alamance Creek (Lake Mackintosh). Please
verify and include in the NOI if necessary.
B1. Please enter the company name as it's registered with the NC Secretary of State. There are a couple "LeoTerra
Development" entities listed and it's not clear which one is supposed to be the permittee.
D9. Make sure the correct permittee name is also listed on the NOI Certification Form.
Part E. The same person who signs the NOI Certification Form must sign the electronic signature section in Part E. No
authorized individual is listed on the NOI Certification Form, so the signature is assumed to be Mr. Lyons'. If Mr. Bunker
