Inspection Summary:
On March 21, 2022, MRO staff performed a Compliance Evaluation of the Former Cash and Carry GW remediation system
under Permit# WI0300466. This system was formerly operating under permit WQ0034454. The recent change of the 02T
Statues places groundwater remediation systems formerly under Non -Discharge to be classified as UIC type permits due to
the presence of an Infiltration Gallery.
O&M records indicate the effluent flow meter is past due for calibration. Calibration of effluent flow meters is to be performed
annually as a condition of the permit.
During the site visit, the O&M records were not present in the treatment trailor. O&M records were emailed to MRO for
review. A copy of the O&M records and the permit should be present and available for review. It is suggested that at least
one year of analytical data also be kept on -site for review.
The groundwater treatment system is permitted by the Division of Water Resources (DWR), any system shut -downs or
system compliance issues need to be reported to the DWR Regional Office within 24 hours of first knowledge that a system
failure or non-compliance issue has occurred. The MRO is requesting that we are copied on all laboratory analytical data for
GW samples collected from this treatment system.

Comment: The system was recently cleaned on January 24, 2022. An O&M record review from January
2022 through March 21, 2022 documents system maintenance activities.
There are no records of effluent flow meter calibration. A new flow meter was installed on
2-27-2021. A calibration sticker should be present on the effluent flow meter to display
calibration is being maintained.
