Division of Water Resources staff person Caitlin Caudle completed a review of the 2021 Residuals Annual Report. This reviev
reflected compliance with the subject permit.
Field CS14-3 and PS2-2 required lime but none was reported to be applied.
Per Permit Condition 111.3 "the Winston Salem Office shall be notified ... at least 24 hours prior to conducting any land
application activity. Such notification shall indicate, at a minimum, the anticipated application time, field IDs, and location of
land application activities. If it becomes necessary to apply to additional fields due to unforeseen events, the Regional Office
shall be notified prior to commencing the application of those fields." Moving forward, please be sure to include all
anticipated application fields in land application notifications.
