Safety/MOT/Traffic Control: Environmental: General Comments • Resident office will be liaison between Webber and Division Environmental staff (i.e. Amy Euliss and Phil Suggs). Division staff will then in turn be the liaison between the project staff and the agencies. • Permit drawings override all project plans as these are what the permit was approved under. If there are discrepancies, this needs to be communicated and clarified prior to work taking place at each site. 404 Permit USAC—Andy Williams • Special condition #1— Need to ensure last modification is included. • USAC noted that Webber and NCDOT staff need to acquire original permit (January 22, 2018) to obtain all conditions. Modified permits do not contain all necessary conditions. • Borrow/Waste pits — Webber needs to submit borrow/waste site locations and plan to USAC if location is within 400' of a stream or wetland. It was noted that this is not for approval purposes, just to keep them in the loop. • USAC needs to be notified prior to work beginning. • Agencies noted that a copy of the permit needs to be kept on site at all times. • Perform erosion control inspection per NPDES requirements. • Take all necessary measures to prevent fresh concrete from contacting live waters. • See Table 8 for list of pipes NOT to be buried for this project. This information is also notated on the plans. (This table was not in the original contract) • There were no cultural resources identified for this project. If any artifacts, graves, or bones are found during excavation, stop work immediately and notify Amy Euliss. 401 Permit DWQ — Dave Wanucha • Most of the conditions are reflective of 404 permit. If we remain compliant with 404, then we will be fully compliant within 401. • NPDES Inspections — Once a week or every 1" of rainfall. • Emphasis was placed on BMPs. Keep equipment out of wetlands and streams. There are BMPs for this as well as dewatering. • Helpful suggestion was mentioned to install a sign at each permit site for quick reference purposes. This would be helpful for agency inspections as well, as they would not be as familiar with the project site. • Noted that there is one permit site that is shared with U-2579AA permit that we CANNOT work in until the permit modification has been approved (Site 14B and Site 31) • A plan for pond draining operations needs to be submitted for approval prior to work taking place. • Equipment Inspections — Inspect equipment regularly for maintenance issues, specifically leaking fluids. • Reminded of 24 hr. deadline to report losses/ violations to DWQ. These need to be reported by Amy Euliss or Phil Suggs. Special notes — Amy Euliss • Wetland areas that fall within slope stake limits still need to be delineated, protected and maintained accordingly until fill is to be placed. • Pond Draining o Plan must be submitted prior to operation. o There is specific language associated to this operation. o If a modification to the channel is necessary, it can be submitted in the draining plan. o Fish CANNOT be transferred or released down stream for potential contamination purposes. The pond will need to be slowly drained, fish collected within pond and then buried to adhere to the requirments. • For culvert site (or any site where pump around is needed) dikes and pump around operation need to be in place even if/when stream is dry. This is to account for high water events in which water will be present. IF not in place, the project is out of compliance. • Permit site details — There is a significant amount of variations to details dependent on which site operations are taking place. All parties need to place special attention to the details called out in the permit drawings. • Special attention was brought to flood plain and channel elevations. o Existing stream bed elevations need to be verified prior to work taking place. If there has been any change to the elevation greater than 6", Amy Euliss needs to be contacted. o Noted native material language for overflow barrels. Also, noted to be aware of requirements for supplementation to the native material with Class B rip rap. • DIAMOND GRINDING SLURRY PLAN o Regardless of when operation is scheduled, the slurry waste plan needs to be submitted as soon as possible. This will allow any necessary modifications to be worked out prior to the work taking place. Please do not shelve this until the operation is impending. • Special discussion was had regarding petroleum spills on the project. o Specific language was referenced and can be found on the proceeding bullet point. o From G.S.143.215.85: A person who owns or has control over petroleum that is discharged into the environment shall immediately take measures to collect and remove the discharge, report the discharge to the Department within 24 hours of the discharge, and begin to restore the area affected by the discharge in accordance with the requirements of this Article if the volume of the petroleum that is discharged is 25 gallons or more or if the petroleum causes a sheen on nearby surface water or if the petroleum is discharged at a distance of 100 feet or less from any surface water body. If the volume of petroleum that is discharged is less than 25 gallons, the petroleum does not cause a sheen on nearby surface water, and the petroleum is discharged at a distance of more than 100 feet from all surface water bodies, the person who owns or has control over the petroleum shall immediately take measures to collect and remove the discharge. If a discharge of less than 25 gallons of petroleum cannot be cleaned up within 24 hours of the discharge or if the discharge causes a sheen on nearby surface water, the person who owns or has control over the petroleum shall immediately notify the Department.
