The Division of Water Quality may require modification or revocation and reissuance of the certificate of coverage. Directions to Site: From Winston-Salem take Thomasville road south and then NC Hwy 109 South for appx.12 miles to Ledford Middle School and turn left on Old Greensboro Road (some maps list it as West Lexington Avenue), continue for 0.8 miles till Old Greensboro Road turns left while West Lexington Ave. continues to right. Follow Old Greensboro Road and then immediately take another left at Mock Road, cross the Abbotts Creek bridge and continue for another %2 mile and turn left on Johnson Road. Continue for 1/4 mile and just before crossing bridge take the access road to the site on the left. 6. Describe receiving stream features and pertinent downstream uses: Spurgeon Creek flows to Abbotts Creek which flows to Tom-A-Lex Lake which is the primary water supply for Lexington and Thomasville. Please provide a description of existing or substantially constructed wastewater treatment facilities: This operation is primarily to collect sand that is sold to asphalt plants. The sand is taken from the stream by dragline, and stored in piles appx. 10 feet from the stream bank. There is a small berm along 80-90% of the stream bank to prevent runoff. The current site topography will direct most of the sites discharge to the middle of the site where a small ditch collects the water before it returns to the creek. A small portion of the surface water will drain to the north end of the site before it gains access to the creek. Only a limited portion of the water draining from the sand piles will return as discharge since most of it will soak into the sandy floodplain area. The surface area should be as large as possible to insure sedimentation occurs. To secure optimum efficiency the flow length of the basin to the basin width should have a ratio of 2:1 • Note: Construction of any wastewater treatment facilities requires submission of three (3) sets of plans and specifications along with the application. Design of treatment facilities must comply with requirements of 15A NCAC 2H .0138. This includes tracing the pathway of a storm sewer to its discharge point. c) If this application is being submitted by a consulting engineer (or engineering firm), include documentation from the applicant showing that the engineer (Or firm) submitting the application has been designated an authorized Representative of the applicant. d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction". e) Final specifications for all major treatment components (if applicable). The specifications must be signed and sealed by a North Carolina registered Professional Engineer and shall include a narrative description of the trey ment system to be constructed.
