George Smith of the Winston-Salem Regional Office of the NC Division of Water Resources (DWR)
conducted a compliance evaluation inspection (CEI) at the subject facility on April 29, 2015.
The facility is located at 4178 Hwy 421 N. It is a small single family home on the right side when
travelling north.
Permit
The certificate of coverage and authorization to construct became effective on September 19, 2013. The
permittee is required to meet all terms and conditions under General Permit NCG550000.
Currently, there is not an engineering certification completed for this system.
Site Review
The grounds are well maintained.
The white 4-inch PVC discharge pipe was on the side of the bank. The pipe discharges into a grassed
roadside ditch, and travels about 100 yards before reaching a culvert under Hwy 421. The effluent will
never reach the surface waters designated as Brushy Fork, Class C waters.
This is a UV disinfection system and does not require total residual chlorine monitoring.
There is a Septitech control box on the site. A red light, which is labeled "Reset" was illuminated.

1. Verify that a UV -disinfection system was installed.
2. Explain why the red light was illuminated.
3. Submit a copy of the engineering certification.
