This letter is in regards to the Notice of Violation received for Woodview Apartments
and the pump station equipment failure. The day of the incident which was February 23,
2022 our countractor was contacted immediatley and remedial actions were taken.
I am including the contract that we hold with CM-1 Spetic Solutions showing that they
provide emergency services such as this incident.

This Service Agreement between CMH Septic Solutions and (N�61tld
is valid for one year, beginning at signing date. CMH Septic Solutions agrees that this service
agreement will consist of ail the following services.

Objective:
Themaintenance plan is a cost effective solution to prolong the life of your lift station, keep It
performing at its highest rate, and decrease the risk of failure of different components, whether
components_are under warranty or not. In addition, it keeps your lift station in good working
condition and allows for fix budgeting for the maintenance of the system which may be
expensive in some cases.
1. Bi- Weekly Inspection
- Check baskets (3) for debris and emptying them
- Check wastewater level for appropriate operating level
- Check alarm system
- Check and clean area around lift station
- Turn on pumps manually to check operating status
Cost: $300 monthly
2. Yearly Inspection
- Pump lift station out
- Wash down lift station (Inside and out)
- Check baskets and basket components (rail system)
- Check pumps for debris
Check alarm controls and floats system
Check and clean area around lift station
Cost: $650 once a year
Extras: Emergency call out/repair: $125/hr. (cost of any supplies will be added)
Emergency pump out and clean-up: $400 (repair cost per hr. will be added for any
repairs)

By.signinfs, client, agrees to pay fQr any of the services that are billed to them. This contract Is
only good for one year frorn the date it Es §igned. Client must sign a new Contract to continue
servites. If contract Is not signed at time of renewal, services will be discontinued.
