No New expiration date: I September 30, 2026 FACILITY OVERVIEW: This facility is 100% domestic waste and serves a mobile home community and adjacent homes and provides treatment for a population of 700. The facility has a permitted and as built flow rate of 0.07 MGD. The treatment system includes the following wastewater treatment components: • Influent pump station • Dual clarifiers • Bar screen • Tablet chlorine disinfection with contact chamber • Flow splitter • Tablet dechlorination • Dual aeration basins • Sludge digester APPLICATION • The treatment facility is operated by Apex Companies, LLC. • Application lists Facility location as 5418 Country Club Road but this doesn't appear to be the location of the WWTP, it is an office/club house area. The contract operator confirmed that the location for the treatment plant should be 215 E. Sheraton Park Road. This address contains the WWTP and Wood Lake. Application lists three year average flow of 0.59 MGD which is around 85% of the permitted flow. The data analysis below is for the almost five year time period of the permit and shows a lower average flow. This would indicate an increase in flow occurring over time at the facility. Application notes the facility serves a population of around 700. Does not indicate if it is just the mobile home park or if it includes adjacent homes. Previous renewals noted that another subdivision was also served by the system. The contract operator confirms that the WWTP serves both the Woodlake and Greybridge communities. INSPECTION The most recent inspection (08/21/2018) did not indicate any major issues. The report did note a dead battery on the auto -dialer and that the generator should be checked monthly and documented in 0&M documents with other records. REVIEW OF PERMIT LIMITS: Flow - The facility is operating on average at around 85% of the flow allowed by the permit over the last three years as reported in the application. Only two monthly average flow values have exceeded the permit limit during the permit cycle. Average flow appears to have leveled off some over the last year after some high flow months in late 2019 and early 2020. Woodlake WWTP Fact Sheet NPDES Renewal 2021 - December 15, 2021 Page 2 Ammonia Nitrogen Limits - There do not appear to be any major issues with no DM and six MA limit violations over the permit term. Fecal Coliform Limits - There was only one DM exceedance over the permit term. BOD Limits - Limits violations were largely found in this parameter. Over the permit cycle there were forty-two (42) DM violations and eight (8) MA exceedances. Compliance issues and enforcements were mainly associated with this parameter. Dissolved Oxygen Limit - There were no limit violations for DO at the effluent. The data does show a number of low values (34%) upstream of the treatment facility. Obviously these issues are not related to the treatment system. Both the effluent and downstream values are consistently at or above 6 mg/L. COMPLIANCE: During the permit cycle this facility had four (4) NODs, and twenty-three (23) NOVs. The notices were mainly for violations of limits for BOD and NH3-N as noted above and a few for Fecal, Flow and TSS. There were seven (7) enforcement actions over the permit cycle. The total amount paid in these enforcements was $3,786.91 with $3,050 in penalties along with $450 in remitted penalties. FILE HISTORY REVIEW SUMMARY • 1977 Waste Load Allocation (WLA) - Established for 0.113 MGD with 7Q10=0. Limits set: BOD -6; NH3-N - 2; DO -6; TSS - 30; Fecal- 1,000; pH 6 to 8.5 - monthly average. • 1979 WLA - for flow of 0.140 - all values same as 1977 WLA. • 1984 WLA - for flow of 0.14 MGD as in 1979. This assessment established Summer/Winter monthly average limits as: BOD - 15/18; NH3-N - 5/10; DO - 6/6; TSS - 30/30; pH - 6 to 8.5 / 6 to 8.5; Fecal - 1,000/1,000. • 1988 Denial of Expansion Request - Expansion request denied due to non -concurrence by Division of Health Services (PWS). Includes WLA for expansion from 0.07MGD to 0.120MGD. • 1990 WLA - 0.070MGD flow. Summer and Winter monthly average limits for first three years of the permit same as 1984 except pH - 6-9/6-9 and Fecal - 200/200. After year three limits for BOD recommended at 5/10 and NH3-N - 1/1.8. All other limits the same. The WLA also include toxicity testing for the first three years and instream monitoring for Temperature, DO, Fecal and Conductivity. The WLA also recommended a letter on potential Chlorine limits in the future. The WLA also recommended removal of the discharge when an alternative was available and requirements for submittal of an engineering report for compliance with the permit. 1990 Staff Report also included and mentions work on the treatment system ongoing in accordance with 1989 authorization to construct (ATC). • October 15, 1991 - Letter to Owner responding to their questions about limits and zero flow designation. • 1993 Staff report - Flow averages around 0.045MGD. Rating sheet showing Class II included. • March 15, 1994 Authorization to Construct (ATC) - Authorized construction of dual tertiary sand filter system, and ultraviolet disinfection system along with removal of chlorine disinfection system. • 1996 Staff Report - Notes changes associated with 1994 ATC on hold as review of possible connection to sanitary sewer under way. Notes BOD concerns and concerns from Public Water Supply as well as impacts and complaints on instream conditions. Region recommended reissuance with requirements to install systems from 1994 ATC and requirements to connect to sanitary sewer when available. • 2002 Permit renewal - Looks like this was the first renewal since 1991 and permit held while there were ongoing reviews of possibilities for connection and concerns from the PWS. Reviews found that Greensboro and Guilford County had no plans for sewer extension around the facility. 1993 Engineering Alternatives analysis included in the package. Documents note that a portion of area served by the system sold to another company and that reviews under way to sell system to a Utility Company. Permit issued with monthly average and daily max summer and winter limits for BOD - Summer 5.0 MA/ 7.5 DM and Winter Woodlake WWTP Fact Sheet NPDES Renewal 2021 — December 15, 2021 Page 3 10/ 15. NH3-N had MA only limits of Summer - 2.0 and Winter 4.0. Other limits were not seasonal - TSS - 30 MA/ 45 DM; Fecal - 200/400. Monitoring also included for Total Nitrogen, Total Phosphorus, pH, Temperature and DO. Temp and DO also required U,D monitoring. Flow limit of 0.070MGD. Permit also included a requirement for development of a Wastewater Management Plan within 180 days. • January 31, 2005 - Name change from Oakwood Land Development Corp to Woodlake utilities, LLC. • 2006 Permit Renewal - NH3-N limits modified to Summer - 2/10 and Winter - 4/20 presumably due to the BAT for small treatment plants. Also included a TRC limit of 17 to begin in April 2008. Permit included special condition for Wastewater Management Plan to be completed by April 1, 2007. Application indicated average flow of 0.047 MGD and that the facility served 306 homes. • 2007 Permit Modification - changed downstream sample location changed due to safety and accessibility concerns. • June 25, 2009 - Ownership change to ARCML06 LLC - Woodlake MHC. • 2011 Permit renewal - Limits remained the same. Wastewater Management Plan requirement included again with due date of March 29.2012. Permit added special condition for Nutrient Re -Opener. Review notes that new owner and new ORC have alleviated a number of compliance issues. Also notes that the permittee receives wastewater from adjacent subdivision where it does not control the collection system. • March 29, 2012 - Submittal of Wastewater Management Plan from Apex Companies. Plan includes a number of maintenance items that will be completed within the next year. Inspection reports note that the facility was under a settlement agreement that included a number of related repairs. All these were handled before the end of the settlement agreement in December 2014 (see 2014 and 2015 inspection reports). • June 20, 2012 - Letter from Apex Companies transmitting Collection System map for the MHC. • October 24, 2012 - Ownership Change to YES WL Utilities EXP, LLC. • 2016 Permit Renewal - Only change is the addition of Electronic Reporting Requirements. Application notes average flow of 0.049 and service area o 330 units (215 at Woodlake and 115 at Greybridge) PROPOSED PERMIT CHANGES: • Updated eDMR requirements to be consistent with final EPA rule. • Added regulatory citations throughout permit as needed. • Corrected locations address for the treatment system in permit. • Updated permit map with additional descriptive information and to make outfall lat/long consistent with location reported on the application. • Updated Nutrient Monitoring Re -Opener language. However, we just noticed one issue that we need to get your input on as soon as we can. This is related to the appropriate owner for the facility. The application for Woodlake WWTP indicates the owner is YES WL Utilities EXP, LLC. This isn't consistent with the application and permit that was issued for your other facility (Autumn Forest). That Facility listed the owner as YES AF Utilities EXP, LLC. Looking at the Secretary of State website it doesn't appear that there is a valid NC Company named YES WL Utilities EXP, LLC. Could you please verify the appropriate Owner name for the Woodlake WWTP facility?
