Dear Mr. Chappell:
In accordance your request for an NPDES permit renewal received on July 20, 1997, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this
decision shall be final and binding.
Please take notice that this permit is not transferable except after notice to the Division of Water
Quality. Part II, E.4. addresses the requirements to be followed in case of change in ownership or control
of this discharge. The Division of Water Quality may require modification or revocation and reissuance of
the permit.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

In compliance with the provision of North Carolina General Statute 143-215.1,

Perquimans County Water Treatment Plant #2
on NCSR 1220
east of winfall
Perquimans County
to receiving waters designated as an unnamed tributary to Mill Creek in the Pasquotank Riyer
Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IV hereof.
This permit shall -become effective March 1, 1998.
This permit and authorization to discharge shall expire at midnight on February 28, 2003.

Perquimans County Water Department
is hereby authorized to:
1. Continue to operate a filter backwash system and a brine recharge softener system located at
Perquimans County Water Treatment Plant #2, on NCSR 1220, east of Winfall, Perquimans
County (See Part III of this Permit), and
Discharge from said treatment works at the location specified on the attached map into an
unnamed tributary to Mill Creek which is classified C-Swamp waters in the Pasquotank River
Basin.

background conditions, the discharge level cannot cause any increase in the turbidity in the receiving water.
All samples collected should be of a representative discharge.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

WLA had water classified as SC and thus gave them Turbidity at 25 NTU (should
be 50 NTU for C-Swamp)
Compliance data: No violations, but all values for the following have been the same every
month for the last two years: Q = 0.035, chloride = 72.0, C12 = 0 and iron = 120.0
Chloride is listed as 72.0 mg/1 on compliance sheet and as 72.0 ppm on DMR.
• Facility is monitoring for TSS monthy instead of bimonthly. *
• Facility is not monitoring for settleable solids. *
• No Turbidity monitoring; weekly U&D required. ORC will start this.
• The data for chloride (72 ppm) is where the operator is writing Chlorine on the
DMR's. He has a column for chloride and one for C12 Difference is?
Chlorine should be written as chloride

ORC will start monitoring for turbidity, settleable solids and TSS bimonthly. He stated that
he will start using exact #'s on the DMR instead of rounding to the same ones every time.

Letter dated 5/4/94 from Lyn Hardison to Russ Chappell told him that he was non-
compliant for not monitoring for everything on his permit.
(I will have compliance unit look into this.)
