The assistance and cooperation of Nick Slogick was greatly appreciated. Details of the inspection are recorded on the attached EPA Water Compliance Inspection Report. Nick Slogick was present for the inspection. Results of the inspection are summarized in the following paragraphs: 15A NCAC 02T .0403 Permitting By Regulation (a)(1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface waters and to prevent any contravention of groundwater standards or surface water standards. Observations: The collection system is very well maintained. Compliant: Compliant (a)(2) A map of the collection system shall have been developed and shall be maintained. Observations: Map displays entire collections system. However, it only reveals pipe location and size, in addition to pump station location. Map does not have information on pipe material, age, and flow direction. Maps need to be updated to contain this information. Complaint: Non -compliant (a)(3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule, spare parts inventory, and overflow response shall have been developed and implemented. Observations: The O&M and spill response plan is very comprehensive and complies with the regulatory standards. Compliant: Compliant (a)(4) Pump stations that are not connected to telemetry system shall be inspected by the permittee or its representative every day, 365 days per year, unless the permittee demonstrates that the daily inspections are not necessary because the pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry system shall be inspected once per week. Observations: All pump stations are telemetry -based systems (3 — radio/2-dial up). They are inspected at weekly with detailed records kept of any service, inspection, testing and maintenance. Compliant: Compliant (a)(5) High -priority sewers shall be inspected by the permittee or its representative once every six months, and inspections shall be documented. Observations: No HPLs in system. Compliant: NA (a)(6) A general observation by the permittee or its representative of the entire collection system shall be conducted once per year. Observations: Permittee inspects entire collection system on an annual basis by breaking areas down into 12 blocks. Each block is inspected for that month; therefore, each block is inspected on an annual basis. Compliant: Compliant (a)(7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC 02B .0506(a), and public notice shall be provided as required by G.S. 143-215.1 C. Observations: No overflows or bypass have been reported in the past 3 years. Compliant: Compliant (a)(8)(B) Grease Control Program shall be in place for private owned collection systems that will include bi-annual distribution of grease education materials to users of the collection system by the permittee or representative. Observations: Permittee distributes bi-annual FOG education newsletters to residents. FOG information can be found on community's website as well. Grease traps are located at the retirement community and club house. They are pumped monthly and records are kept. Observations: Easements are maintained properly by community's landscaping service. Records are kept of service. Compliant: Compliant (a)(10) Documentation of compliance with Subparagraphs (a)(1) through (a)(9) of this Rule shall be maintained by the collection system owner for three years with the exception of the map, which shall be maintained for the life of the system. Observations: Excellent recording keeping of collection system. Detailed and comprehensive. The only exception is the map of collection system needs updates to include pipe age, material, etc. Compliant: Compliant Special Note concerning upcoming merger with Davie Co Utilities: Mr. Lee Rollins, Town Manager, emailed inspector information concerning FTSE: "The Town of Bermuda Run applied for a "fast track sewer system extension application" on February 10, 2022. The purpose for the application is to decommission the town's existing package wastewater treatment plant, upgrade the existing pump station at the plant and construct a force main to tie into the Davie County Public Utilities pump station. The sewer flow would then be pumped to the Muddy Creek WWTP owned by the Winston-Salem/Forsyth County Utilities Commission. The origin of wastewater in the town's collection system is 95% residential and 5% commercial (Bermuda Run Country Club clubhouse and Bermuda Village Retirement Community). Repairs are made to the collection system as needed. The town intends to work toward turning the collection system over to Davie County Public Utilities and is currently part of a master agreement with Hazen & Sawyer through Davie County Public Utilities" Page 3 of 7 Permit: WQCSD0259 Owner - Facility:Town of Bermuda Run Inspection Date: 02/24/2022 Inspection Type : Collection System Inspect Non Sampling Reason for Visit: Routine General # Is this system a satellite system? # If Yes, what is system name or permit number? The map shows locations and size of pipes, pump stations and WWTP. It has not been updated to include pipe material, flow direction or pipe age. All system maintenance records are maintained well and comprehensive. Grease Control Is grease/sewer education program documented with req'd customer distribution? # Are other types of education tools used like websites, booths, special meetings, etc? If Yes, what are they? (This can reduce mailing to annual.) Mailings are sent out to customers via community newsletter and can also be found on community website. For public systems, is there a Grease Control Program via an ordinance/agreement? If Yes, does it require grease control devices at applicable locations? Boxwood Circle 2. Main (goes to WWTP and will soon be major connection when Davie Co Utilities acquires control of collection system). Juniper Circle will be the site a new force main connecting to Davie County Utilities. All pumps stations are connected to telemetry system. Three pump stations have radio (making it possible to keep track of alarms online) and two stations are 'dial -up' (call out). # Number of duplex or larger pump stations in system # Number of vacuum stations in system # Number of simplex pump stations in system # Number of simplex pump stations in system serving more than one building How many pump/vacuum stations have: # A two-way "auto polling" communication system (SCADA) installed? # A simple one-way telemetry/communication system (auto -dialer) installed? Generators are tested every 6 months and properly documented. Natural gas generators are located at Ivy Circle and Boxwood Circle stations, Propane generator at Riverhill station, Bermuda Village has the only portable 185 generator and the Main station is diesel. 5 pump stations total in collection system (4 duplex / 1 triplex).
