USA, Permit No. CM-33 Glenmark was granted an A to C on April l st 2021 to upgrade pretreatment system (a copy of the A to C is enclosed). Glenmark Pharmaceuticals Inc. USA was SNC for the first six-month period of 2021. The facility has been listed in a public notice in The Enquirer Journal and was assessed a civil penalty of$500.00 They were under a Schedule of Compliance from August 5th 2021 to December 31 th 2021 (see SOC copy enclosed). Pilgrim's Pride Corporation, Permit No. CM-28 was issued a Schedule of Compliance for interim limits to allow them to remove sludge from lagoon one (a copy of SOC is enclosed). NC Biogas Permit No. CM-35 Construction for the project was delayed due to Covid-19, construction is expected to begin 2022. There was zero discharge for the year of 2021. Permits were renewed/modified and approved for Atrium-Union CM-14, Glenmark Pharmaceuticals, Inc. USA CM-33, NC Biogas CM-35 and Pilgrim's Pride Corporation CM-28. Monroe,North Carolina 28110 II AUTHORIZATION TO CONSTRUCT PRETREATMENT SYSTEM INDUSRIAL USER PERMIT NUMBER CM-33 Dear Ms. Knight: Based on general review of the information that has been submitted for Glenmark Pharmaceuticals Inc. USA. Authorization is hereby granted for the modification of the wastewater pretreatment system. The modification will include: • Installation of one 10,000 gallon holding tank. • Installation of two heat exchangers and one water chiller. • All associated valves,piping,pumps and appurtenances. This authorization to construct is issued in accordance with PART II 26 of Industrial User Permit CM-33, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in IUP CM-33, and in accordance with the plans submitted for review. A copy of the authorized plans and specifications shall be maintained on file at the facility by the Permittee for the life of the facility. Proper design and construction of these pretreatment facilities, and satisfactory operation to comply with all limits of IUP CM-33, is the ultimate responsibility of Glenmark Pharmaceuticals Inc. USA. In the event that the approved pretreatment system fails to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective actions and notify the City of Monroe. Glenmark has been unable to consistently meet permit limits contained in CM-33. 3. Glenmark has proposed a corrective action plan whereby pretreatment system improvements will be constructed to provide a long term solution to meeting all required permit limits. 4. Glenmark will complete activities identified in the corrective action plan to meet permit limits in accordance with the following milestone: a. Complete pretreatment system installation,testing, and commissioning by October 31,2021. Installation of equipment includes a flow equalization tank,two heat exchangers and a chiller. 5. Glenmark shall submit progress reports to the City within 14 days of the completion of milestone shown above. 6. To assist Glenmark in completing the corrective action plan and to comply with all conditions of their permit,this Schedule of Compliance is hereby established and will remain in effect from the execution date until December 31,2021. During this time period, Glenmark shall meet and comply with all terms of their current Industrial User Pretreatment Permit No. CM-33.
