LP has operated in compliance with the permit requirements during 2021 except as noted below. Groundwater Monitoring—Monitoring, Report Keeping, and Reporting LP has six (6)monitoring wells listed in Permit No. WQ0000461. These wells were sampled twice (2)per year according to the requirements of the current permit. The six (6) wells are located on LP property and correspond to selected groups of land application fields that are monitored. All land application ceased in the monitoring area of field 1228-10387-25 in 2008. LP has indicated non-compliance with this requirement due to elevated nitrate readings exceeding 10 mg/l measured in groundwater monitoring wells DSI-MW1, DSI-MW5, and DSI-MW9 during the March and September monitoring events. The results of the groundwater monitoring are included in this report in the Groundwater Analysis section. 1068 ABTCO Road ADDRESS North Wilkesboro,NC 28659 336.696.2751 TEL 336.696.3443 FAX vmv.lpcorp.com WEB BUILD WIT$ US.'
