Dear Ms. Jefferies:
On February 7, 2022, Division of Water Resources staff Caitlin Caudle completed a
scheduled compliance inspection of the subject facility. Carl Cheek, ORC, was present
during the entire inspection. A review of the facility was completed, and a separate
electronic review of available records was completed. Due to weather conditions the
facility was not reviewed in operation. This review reflected non-compliance with the
subject permit.
The following items must be completed for the facility to be compliant with the subject
permit:
1. It was unclear whether the previous operator finished calibrating the irrigation
system as required by Permit Condition 111.7. Please calibrate the system as
soon as possible.
2. Flow has not been recorded properly since November as required by Permit
Condition IV.3. Ms. Caudle explained that influent flow must be measured as the
monthly average of the water usage, which can be calculated using the water
meter. Please begin recording flow as discussed and report those values on
the Non Discharge Monitoring Reports.
3. Monitoring reports have not been submitted since July 2021. Please submit
monitoring reports for November 2021 onward. Failure to submit monitoring
reports by the end of the following month is a violation of the permit and can
result in enforcement action.

Inspection Summary:
On February 7, 2022, Division of Water Resources staff Caitlin Caudle completed a scheduled compliance inspection of the
subject facility. Carl Cheek, ORC, was present during the entire inspection. A review of the facility was completed, and a
separate electronic review of available records was completed. Due to the weather conditions the facility was not reviewed in
operation.
Mr. Cheek was designated as the operator on November 1, 2021, after the previous operator abandoned the system. The
previous operator did not provide NDMRs and NDARs or the Operation and Maintenance Plan to Mr. Cheek. Ms. Caudle
provided the most recent copy of the Operation and Maintenance Plan, updated NDAR and NDMR forms, calibration forms,
soil sampling information, a signature authority form, the Laserfiche file, and information to submit monthly reports
electronically.
Brush has been removed from the irrigation zones as referenced in NOV-2020-PC-0407 dated September 10, 2020, and in
the subsequent follow-up inspections. All irrigation zones can easily be inspected now.
The following items must be completed for the facility to be compliant with the subject permit:

Please review the subject permit for all monitoring requirements, operations and maintenance requirements, performance
standards, and records that must be maintained on site. A compliance inspection will be scheduled during the next calendar
year.
Other observations:
• 2020 soils analysis was not available. The 2021 soil analysis was available on the NCDA PALS portal. The PALS# for
the facility referenced is 453193. Please use this number moving forward to better track soil analyses. If assistance and
further guidance with soil sampling is needed, please contact Ms. Caudle.
• Freeboard is not required to be reported by the current permit.
• Mr. Cheek had an electronic copy of the most recent permit.
• Ms. Caudle asked that Mr. Cheek provide her with copies of any lab results, electronic records, and copies of his
maintenance and inspection logs.
• No residuals removal records were available. Please review Permit Condition 111.11 and Permit Condition IV.7 for the
residuals removal requirements.
• Mr. Caudle reminded Mr. Cheek that the contact information in the Operations and Maintenance Plan needs to be
updated.
• Previous NDARs and NDMRs were not present. The previous operator kept these documents and did not provide them
to the facility.
• Flow rates and application rates are below the permitted limits.
• The influent pump station was reviewed from the vehicle. General housekeeping was good. The alarm was not tested.
• The sand filters appeared to be in good condition.
• The tablet chlorinator had tablets in the cylinder and the chamber was clear of debris and other growth.
• The alarm on the irrigation system was operational.
• The rain sensor was present and appeared to be operational.
• Mr. Cheek provided electronic copies of the available inspection and maintenance log, and the irrigation data he has
recorded since November.
• Monitoring reports have not been submitted since July 2021. Monitoring reports could not be provided for August,
September, and October 2021 as the previous ORC did not visit the system and the new ORC did not start until November.
