The North �arolina Department of Tran�portation is scheduled to replace Bridge #442 with a
dc�uble barreled 9'x 5'x 40' RCBC at the same Ioca:tion. The exlsting bridge is 15' long x 18'
wide with all timber camponents. The culvert vvill be buried 1' into the streambed and flt�w will
be isolated to one barrel. Alternating baffles will b� added ta maintain and possibly lmprov�
aquatic life passage. Mltigation will be provided through EEP (40' x 2:1 �. No on-site detaur is
proposed. Stream impacts are tabulated below.

�anstruction of this project will requlre impacts to �vaters o� the �.Jnited �tates �n the Yadk�n
River Basin. The existing bridge crosses the headVVaters of the North �'ork �.eddies River ((�1ass:
WS-II; Trout; �IQW). This proj ect was reviewed for the presence af threate�ed and endangered
species and vvill have no effect on any federally listed threatened or endan,gered species.
Cultural resaurce compliance work has been completed (see attached).
The project impacts should not remove or degrade existing water quality us�s. StQr�nwater will
be rauted through grass or rock lined ditches, depending on the ditch line slape. Total land
dis�urbance will be less than 1 acre. By utilizing the erosion control measures outlin�d 1n the
attachments ta this appllcation, dovvnstream water qualxty standards will be protected. All
apprapriate BMPs will be used during constructio�.

General Project Narrative:This project involves the replacement of Bridge No. 960442 over North Fork Reddies RIver on SR 1501 (Vannoy Road) in WIlkes County, NC. The bridge will be replaced
with a 2 @ 9' x 5' reinforced concrete box culvert. The proposed culvert is 38.7' in length at a skew of 99° to the proposed roadway and has a slope of 3.81%. The proposed
culvert will be constructed with a flood plain bench in order to force low flows through only one barrel of the culvert while allowing high flows access to both barrels of the
culvert. Because of the steep slope of the proposed culvert it will also be constructed with sills and baffles in the low flow barrel in order to promote bed material retention and
to allow aquatic organism passage. Bridge replacement and associated roadway improvements have been designed in a manner to minimize the increase in impervious
surface area to the maximum extent possible. Runoff in the proposed roadway ditch will drain to a narrow slot type D 2GI, which will be carried under the proposed roadway
through a CL III 15" reinforced concrete pipe, and will drain to the North Fork Reddies River from the SW quadrant. No drainage from impervious surfaces will be carried in
the proposed roadway ditch, therefore no treatment is necessary.
