A. Residuals Land Application Program (FORM: RLAP 06-16) Application:
1. No comments.
B. Operation and Maintenance Plan (O&M Plan):
1. In accordance with section E of the RLAP 06-16 application instruction form, an O&M Plan shall
be provided for all new and renewal application packages for review. Please provide the O&M Plan
which meets the 15A NCAC 02T .I 110 requirements.
C. Residuals Source Certification (FORM: RSC 06-16) Application:
1. No comments.
D. Land Application Site Certification (FORM: LASC 06-16) Form:
1. The site map for NC-YK-33 Fields 1 through 3 is not clear and legible. A legible electronic version
of this map shall be provided that meet the requirements listed in the "Map Guidance for Residuals
Land Application Permits" and "Soil Scientist Evaluation Policy" memorandums.
2. There is a habitable residence in the southwest of field NC-YK-33-01 under separate ownership on
parcel 145603 which appears to be within the 400-foot setback from the application field as
required for the surface application of residuals. The submitted site map appears to show that there
previously was a setback waiver for this residence.
This setback shall be shown on the resubmitted site map for this field as required by Item D.1., and
any acreage change shall be included in an updated LASC form. Alternatively, a setback waiver
may be executed in accordance with 15A NCAC 02T .1108(e) to reduce this setback. This waiver
shall be written, notarized, signed by all parties involved, and recorded with the Yadkin County
Register of Deeds.
There is a field ditch present on the south side of the access road at field NC-YK-33-04. This was
previously discussed with the regional office, with the response being that this feature was already
buffered out. However, it appears from the site map that the access road is buffered out, but the 25-
foot buffer for the ditch was not (you can see the ditch in the aerial within the field, unlabeled and
not buffered). This diversion feature shall be shown on a resubmitted site map with the appropriate
setback, and any acreage change shall be included in an updated LASC form.
E. Landowner Aereement (FORM: LOA 06-16) Form:
The LOAs for fields NC-YK-09-01 and NC-YK-35-01 did not indicate any need for a re -defined
compliance boundary for this site. However, the site maps for these field indicate there are wells
on these parcels that could be within the compliance boundary of these field. 15A NCAC 02L
.0107(d) prohibits the construction and operation of water supply wells within the compliance
boundary. Please indicate whether the wells on this field are water supply wells. If so, then Section
III of the LOA for this field shall be edited to request a re -defined compliance boundary for this
field in accordance with 15A NCAC 02T .0105(h) with the rationale for the requested re -location
of the compliance boundary.
