Hi Tony,
Following up our phone conversation with an email to reiterate that the entity/person listed on the FRO form must also be
the NCGO1 permittee (the entity/person listed in B1 on the e-NOI). In this case, that means Mr. Kizior must be the
NCGO I permittee. If you want to redo the FRO form and have Parsons General Contractors listed as an additional
financially responsible party, then Parsons can definitely be the NCGO1 permittee.
