Apex Companies, LLC dba Maryland Apex Engineering, PC (Apex) is pleased to present this
proposal to the City of Concord to complete the maintenance and repairs of all on -site stormwater control
measures at the location listed above. The scope of work below includes deficiencies that were located
during the inspection of the two on -site bioretention stormwater control measures (SCM), that was
conducted by Apex on June 5, 2019. No work other than that described below will be completed onsite
without prior approval.
This proposal does not include repairs to the existing facility other than those outlined in the scope
of work. In the event that further inspection reveals the need for additional work, Apex will notify the City
of Concord immediately and provide an estimate to complete that work.
If the outlined terms are agreeable, please sign and return the attached Proposal/Contract for
Services to indicate your acceptance. Receipt of the signed Proposal/Contract for Services will constitute
formal notice to proceed.

SCOPE OF WORK:
Task 1 — Inlet/Structure Cleanout
• Mobilize personnel to the site;
• Document existing conditions;
• Using a vacuum trailer, the drop inlets, curb inlets, junction box, and control box holding sediment
and or vegetation will be cleaned out.
• Apex will remove the sediment and vegetation collected out of the structures and load it into a 10-
yard dumpster to be disposed of offsite. Depending on the timing of the project, the cut vegetation
will either be disposed of at a landfill or a wood waste recycling facility.
• All areas disturbed during the completion of this task will be seeded and covered with a combination
of mulch, straw, and erosion control matting so final stabilization can be achieved.
Task 2 — Sediment Removal and Sinkhole Repair
• Using a mini excavator, the accumulated sediment in the riprap channels will be removed;
• Riprap stone will be added as needed to slow water velocity;
• The sinkhole next to the control structure will be excavated to find the cause;
• If the cause of the sinkhole can be located and no parts are needed, it will be repaired using
hydraulic cement;
• All areas disturbed during the completion of this task will be seeded and covered with a combination
of mulch, straw, and erosion control matting so final stabilization can be achieved.
Task 3 — Manhole Ring Repair
Using a hammer drill and concrete anchor hardware, two manhole rings will be permanently
attached to the SW box;
Clean and demobilize from the site; and
Submit a work completion and reinspection report to the City of Concord.
Cost Proposal
We propose to complete the Scope of Work described above on a lump sum basis using the rates described
below. All work will be conducted in accordance with the Terms and Conditions of the contract between
Apex and the City of Concord. Our cost proposal is based on the following assumptions:
- Apex will have free and unimpeded access to the work location;
- Work is limited to three days onsite;
- This project is weather dependent for scheduling purposes.
- Up to 5 tons of 6 to 8-inch riprap stone will be added to the structures as needed;
- Apex will install seed, mulch, straw, and erosion control matting as specified above;
- The proposal includes weekly watering of the restored areas for a period of up to six weeks or
until grass becomes established and assumes that water is available from the facility. If water is
not available there will be an additional charge of $150/week.
- No permits are required to complete outlined work scope.
