Dear Mr. Witherspoon:
The Division of Water Resources (Division) Central Office and Regional Office staff have
reviewed Louisiana Pacific Corporation's Groundwater Investigation Report and Corrective
Action Plan (CAP) submitted on May 14, 2021. The Division recognizes that Louisiana
Pacific Corporation submitted their Groundwater Investigation Report and CAP in a timely
fashion in fulfilment of Permit Condition I.1. However, both the groundwater investigation
report and the CAP were found to be insufficient for approval due to the following issue:
The groundwater investigation must be completed as a Comprehensive Site Assessment
(CSA). The CSA must be performed by a person, firm or professional corporation duly
licensed to offer geological or engineering services in accordance with 15A NCAC 02L
.0103(e). Additionally, it must follow the guidelines outlined in 15A NCAC 02L
.0106(g). The CAP must also be signed and sealed by a person, firm or professional
corporation duly licensed to offer geological or engineering services and satisfy the
conditions listed in 15A NCAC 02L .0106(h).
Please revise the investigation report in accordance with CSA requirements, ensure that it
has been signed by a licensed professional and addresses each item listed in 15A NCAC
02L .0106(g). Please submit a copy of the revised report to this office within 90 days of
receipt of this letter. After the report has been submitted the Division will provide a written
response as to approval status of the report. Once the investigation report is approved by the
Division, the CAP must then be revised accordingly, it must also be signed by a licensed
professional and address items contained in 15A NCAC 02L .0106(h).
