Dear Mr. Basinger,
This letter is a follow up to our Notice of Violation of the above referenced NOV.
612 East 12th Street (Incident Number 202200065):
• Incident occurred on 12/31/2021.
® The SSO was caused by grease accumulation, which caused a blockage in the
mainline.
® We immediately dispatched a crew out to the location to inspect.
® The sewer main blockage was unstopped the same day with a Jet -Vac combo truck.
® The impacted area near the affected manhole was flushed with fresh water and lime
applied. The impacted area was thoroughly cleaned.
• Immediate repairs were made the same day. The mainline was washed again the
following day and the mainline was inspected via CCTV. No further issues were
observed.
