NPDES SELF -INSPECTION PROGRAM:
EFFECTIVE OCTOBER 1, 2010, PERSONS CONDUCTING LAND -DISTURBING ACTIVITY LARGER THAN ONE (1) ACRE
MUST INSPECT THEIR PROJECT AFTER EACH PHASE OF THE PROJECT, AND DOCUMENT THE INSPECTION IN
WRITING. ALL EROSION AND SEDIMENTATION CONTROL MEASURES, INCLUDING, BUT NOT LIMITED TO,
SEDIMENTATION CONTROL BASINS, SEDIMENTATION TRAPS, SEDIMENTATION PONDS, ROCK DAMS,
TEMPORARY DIVERSIONS, TEMPORARY SLOPE DRAINS, ROCK CHECK DAMS, SEDIMENT FENCE OR BARRIERS, 2
ALL FORMS OF INLET PROTECTION, STORM DRAINAGE FACILITIES, ENERGY DISSIPATERS, AND STABILIZATION 2
METHODS OF OPEN CHANNELS MUST BE INSPECTED BY THE FINANCIALLY RESPONSIBLE PARTY, LANDOWNER,
OR THEIR AGENT.
OFFSITE SPOIL NOTE:
IF AN OFFSITE SOIL SPOIL OR BORROW SITE IS UTILIZED, THEN THE DISTURBED AREA FOR THE SPOIL/BORROW
SITE MUST BE INCLUDED IN THE LAND -DISTURBANCE PLAN AND PERMIT UNLESS THE SPOIL/BORROW SITE
ALREADY HAS A SEPARATE LAND -DISTURBANCE PERMIT.
STAGE 1 SPECIFIC SEQUENCE
1. OBTAIN LAND DISTURBING PERMIT AND PLACARD, AND POST THE PLACARD ON SITE.
C2.PERPDE:REQUIREMENTS, A RAIN GAUGE, SELF -INSPECTIONS RECORDS, PERMIT, CERTIFICATE OFRAGD S&E PLAN ARE REQUIRED TO BE MAINTAINED ON SITE AND ACCESSIBLE DURINGCTIOIS RECOMMENDED THAT THESE ITEMS BE PLACED IN A PERMITS BOX AT THE BEGINNING ORANCROJECT.
3. OBTAIN AN APPROVED (STAMPED) S&E CONTROL PLAN AND KEEP IT ON SITE, EITHER IN THE INSPECTION
BOX, CONSTRUCTION OFFICE OR WITH THE CONTRACTOR.
4. SCHEDULE A PRE -CONSTRUCTION MEETING WITH THE MUNICIPALITY AND OBTAIN A LAND DISTURBANCE
PERMIT.
5. NO TIMBERING OR DEMOLITION OF EXISTING STRUCTURES WILL BE ALLOWED UNTIL PLAN IS APPROVED.
6. MINIMAL CLEARING AND GRUBBING TO INSTALL PERIMETER MEASURES AND CONVEYANCES WILL BE
ALLOWED. STAGED STABILIZATION ON BARE SOILS OF SLOPES AND OTHER COMPLETE OR INACTIVE AREAS
WILL BE DONE IN ATIMELY MANNER AS OUTLINED UNDER NPDES PERMIT CONDITIONS.
7. INSTALL TREE PROTECTION FENCING ACCORDING TO THE APPROVED EROSION CONTROL PLAN. CALL THE
MUNICIPALITY EROSION CONTROL FOR INSPECTION AND OBTAIN PERMIT.
8. INSTALL TEMPORARY CONSTRUCTION FENCE, CONSTRUCTION ENTRANCE, SILT FENCING, SILT FENCE
OUTLETS AND INLET PROTECTION AS SHOWN ON APPROVED STAGE 1 EROSION CONTROL PLAN. GRADE
ONLY AS NECESSARY TO INSTALL THESE DEVICES. INSTALL SILT FENCE OUTLETS AT LOW POINTS IN THE SILT
FENCING AS NEEDED TO PREVENT BLOWOUTS.
9. INSTALL DIVERSION DITCHES/BERMS AND INLET PROTECTION ON EXISTING INLETS AS SHOWN. CONTRACTOR
TO DIVERT RUNOFF TO PROPOSED SEDIMENT TRAP.
2 10. ALL DITCHES SHALL BE LINED TO THE TOP OF BANK.
11. CONSTRUCTION FENCING SHALL BE PLACED ALONG LIMITS OF DEMOLITION. CONTRACTOR SHALL PLACE
AND PHASE AS NECESSARY. ACCESS POINTS AND LIFE -SAFETY EGRESS SHALL COMPLY WITH OSHA
REQUIREMENTS.
12. IF A STOCKPILE IS NECESSARY DUE TO EXCAVATION OF THE SEDIMENT TRAP/BASIN, PLACE A DOUBLE ROW
OF SILT FENCE 10 FEET APART. TREES AND GROUNDCOVER MAY BE REMOVED ONLY AS NECESSARY FOR
THIS STOCKPILE. IF MATERIAL IS GOING TO BE HAULED OFF -SITE, A SEPARATE EROSION CONTROL PERMIT
MUST BE OBTAINED FOR THE DESTINATION OF THE MATERIAL.
13. NOTIFY THE MUNICIPALITY S&E SITE INSPECTOR AFTER MEASURES HAVE BEEN INSTALLED AND PROJECT HAS
STARTED.
14. STABILIZE ALL DISTURBED AREAS AND OTHER CRITICAL EROSION CONTROL AREAS IMMEDIATELY UPON
INSTALLATION.
15. ONCE APPROVED, BEGIN DEMOLITION OF EXISTING PARKING FIELD AND ASSOCIATED STRUCTURES AS
OUTLINES ON THE DEMOLITION PLAN (SHEET C1.01) AND BEGIN GRADING REMAINDER OF THE SITE.
MAINTAIN EROSION CONTROL DEVICES AS NEEDED.

16. ALL CUT OR FILL SLOPES, SOIL PILES, UPSTREAM AND DOWNSTREAM BASIN SLOPES WILL BE COVERED WITH
A SUITABLE RECP AFTER SEEDBED PREP, ADDITION OF SOIL AMENDMENTS AND SEEDING.

STAGE 2 SPECIFIC SEQUENCE

2. MAINTAIN EROSION CONTROL MEASURES WEEKLY AND AFTER EACH RAIN EVENT THROUGHOUT
CONSTRUCTION.
3. CONTRACTOR SHALL MAINTAIN INLET PROTECTION AROUND ALL CONSTRUCTED INLETS AS SHOWN ON PLANS.
4. STABILIZE GRADED AREAS AS THEY ARE BROUGHT TO FINAL GRADE. TEMPORARILY SEED AND MULCH
DENUDED AREAS WITHIN 15 CALENDAR DAYS AFTER ANY PHASE OF GRADING. ONCE FINAL GRADE HAS BEEN
ACHIEVED, PLACE COMPACTED STONE SUB GRADE OR PERMANENTLY SEED ALL AREAS AND SLOPES PER
NCDENR'S EROSION SEDIMENT CONTROL PLANNING AND DESIGN MANUAL VERSION 5/13, CHAPTER 6.11.6.10
AND 6.11.6.11. SEE DETAIL SHEETS FOR SEEDING SPECIFICATIONS.
5. REQUEST FINAL APPROVAL BY CONSTRUCTION INSPECTOR.
6. PERIMETER MEASURE MUST BE LEFT IN PLACE UNTIL ALL UPLAND AREAS ARE PERMANENTLY STABILIZED.
AFTER SITE IS PERMANENTLY STABILIZED, REMOVE ALL TEMPORARY EROSION CONTROL MEASURE AND
PROVIDE PERMANENT SEEDING WHERE TEMPORARY MEASURES HAVE BEEN REMOVED AND GROUND COVER
IS NOT ADEQUATE. SEDIMENT BASINS MAY NOT BE REMOVED OR CONVERTED TO PERMANENT SCMS UNTIL
ALL UPLAND AREAS ARE PERMANENTLY STABILIZED.
7. UPON APPROVAL OF PERMANENT STABILIZATION BY EROSION CONTROL INSPECTOR, REMOVE REMAINDER OF
EROSION CONTROL MEASURES, GRADE AND STABILIZE AND PAVE REMAINDER OF SITE. CONVERT SEDIMENT
BASIN INTO PERMANENT SEDIMENT CONTROL MEASURE AS LAID OUT IN SECTION 16 OF EROSION CONTROL
NOTES.
8. WHEN PROJECT IS COMPLETE, THE PERMITTEE SHALL CONTACT DEMLR TO CLOSE OUT THE E&SC PLAN.

LEAST 3 FULL BUSINESS DAYS PRIOR TO BEGINNING CONSTRUCTION
OR EXCAVATION TO HAVE EXISTING UTILITIES LOCATED.
CONTRACTOR SHALL CONTACT ANY LOCAL UTILITIES THAT PROVIDE
THEIR OWN LOCATOR SERVICES INDEPENDENT OF "NC811 ".
REPORT ANY DISCREPANCIES TO THE ENGINEER IMMEDIATELY.
EROSION CONTROL NOTES:
1. ALL EROSION AND SEDIMENT CONTROL DEVICES SHALL BE PER THE STANDARDS AND SPECIFICATIONS OF
THE MOST RECENT VERSION OF THE NORTH CAROLINA EROSION AND SEDIMENT CONTROL PLANNING AND
DESIGN MANUAL.
2. CONTRACTOR SHALL NOTIFY THE DEMLR RALEIGH REGIONAL OFFICE (919. 791. 4200) AT LEAST 48 HOURS
PRIOR TO ANY LAND DISTURBING ACTIVITY.
3. EROSION AND SEDIMENT CONTROL (E&SC) PERMIT AND A CERTIFICATE OF COVERAGE (COC) MUST BE
OBTAINED BEFORE ANY LAND DISTURBING ACTIVITIES (INCLUDING TIMBERING AND DEMOLITION) OCCUR.
4. APPROVAL OF THIS PLAN BY NCDEQ DOES NOT ABSOLVE THE OWNER/OPERATOR FROM OBTAINING
PERMITS FROM OTHER AGENCIES.
5. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING ALL EROSION CONTROL DEVICES FOR THE
DURATION OF THE PROJECT. ALL EROSION CONTROL DEVICES SHALL BE CHECKED DAILY TO ENSURE THAT
ALL ARE PROPERLY IN PLACE AND FUNCTIONING AS PLANNED. ALL EROSION CONTROL DEVICES WILL BE
REPAIRED (CLEANED) AS NECESSARY, AND WITHIN 48 HOURS FOLLOWING ANY RUNOFF PRODUCING STORM
EVENT.
6. SELF -INSPECTIONS FOR EROSION AND SEDIMENT CONTROL MEASURES ARE TO BE PREFORMED AT LEAST
ONCE EVERY SEVEN CALENDAR DAYS AND WITHIN 24 HOURS OF EVERY RAIN EVENT GREATER THAN ONE
INCH. ANY NEEDED REPAIRS SHALL BE MADE IMMEDIATELY TO MAINTAIN MEASURES AS DESIGNED. ALL ESC
MEASURES SHALL BE MAINTAINED AS SPECIFIED IN THE CONSTRUCTION DETAILS ON THIS PLAN. A RAIN
GAUGE SHALL BE INSTALLED AT THE PROJECT SITE FOR MONITORING.
7. CONTRACTOR SHALL SEED AND STABILIZE ALL STEEP SLOPES (STEEPER THAN 3HAV OR GREATER THAN 50 FT
IN LENGTH) WITHIN 7 DAYS, 14 DAYS FOR MODERATE SLOPES (3H:1V OR FLATTER) .
8. FOR ANY LAND -DISTURBING ACTIVITY WHERE GRADING ACTIVITIES HAVE BEEN COMPLETED, TEMPORARY
OR PERMANENT GROUND COVER SUFFICIENT TO RESTRAIN EROSION SHALL BE PROVIDED AS SOON AS
PRACTICAL, BUT IN NO CASE LATER THAN SEVEN (7) DAYS AFTER COMPLETING THE WORK. STABILIZATION IS
THE BEST FORM OF EROSION CONTROL. TEMPORARY SEEDING IS NECESSARY TO ACHIEVE EROSION
CONTROL ON LARGE DENUDED AREAS AND ESPECIALLY WHEN SPECIFICALLY REQUIRED AS PART OF THE
CONSTRUCTION SEQUENCE ON THE PLAN.
9. NO DEBRIS SHALL BE TRACKED ONTO PUBLIC RIGHT OF WAY. IF THE SITUATION OCCURS WHERE MUD,
ROCK AND DEBRIS IS TRACKED ONTO PAVEMENT, THE CONTRACTOR SHALL CLEAN THE PAVEMENT AND
INSTALL ADDITIONAL MEASURES TO PREVENT THE FUTURE OCCURRENCE.
10. ALL DISTURBED AREAS NOT RECEIVING OTHER PROPOSED IMPROVEMENTS TO RECEIVE TEMPORARY
SEEDING, PERMANENT SEEDING AND MULCH. ALL SLOPES OF 2:1 OR GREATER MUST RECEIVE BLANKET AND
MATTING IN ADDITION TO SEEDING AND MULCHING PRACTICES AS STATED IN THE MOST RECENT VERSION
OF THE NORTH CAROLINA EROSION AND SEDIMENT CONTROL PLANNING AND DESIGN MANUAL.
11. CONTRACTOR TO TEMPORARY SEED & MULCH DIVERSIONS IMMEDIATELY FOLLOWING CONSTRUCTION.
12. CONTRACTOR TO BE RESPONSIBLE FOR REROUTING ALL TEMPORARY DIVERSIONS AND SILT FENCE AS
NECESSARY DURING CONSTRUCTION TO MAINTAIN PROPER DRAINAGE OF DISTURBED AREAS. CONTRACTOR
SHALL BE RESPONSIBLE FOR ANY AND ALL ADDITIONAL E&S MEASURES OR MAINTENANCE REQUESTS OR
REROUTING, OR RELOCATING, E&S MEASURES AS DEEMED NECESSARY BY THE LOCAL AUTHORITY OR
LATEST EDITION OF THE NORTH CAROLINA EROSION AND SEDIMENT CONTROL PLANNING AND DESIGN
MANUAL.
13. ALL FILL AREAS TO BE STRIPPED OF UNSUITABLE MATERIAL BEFORE PLACING AND COMPACTING FILL
MATERIAL.
14. CONTRACTOR SHALL CONTACT ENGINEER IF ADDITIONAL CONSTRUCTION ENTRANCE MEASURES ARE
NEEDED TO PREVENT THE TRACKING OF MUD, DIRT, OR DEBRIS ONTO ANY PAVED SURFACES INSIDE OR
OUTSIDE OF THE PROPOSED CONSTRUCTION LIMITS.
15. UPON COMMENCEMENT OF DEMOLITION ACTIVITIES THE CONTRACTOR SHALL BE RESPONSIBLE FOR
OBTAINING AND SUPPLYING THE NECESSARY SAMPLES AND RESULTS TO DETERMINE FERTILIZER AND
NUTRIENT APPLICATION FOR THE ESTABLISHMENT OF GRASS IN THE SITE.

16. DURING THE INITIAL STAGES OF CONSTRUCTION, THE STORMWATER CONTROL MEASURE MAYBE USED AS
A SEDIMENT BASIN FOR EROSION CONTROL PURPOSES. IF SO, THE CONTRACTOR SHALL FOLLOW THE
GENERAL CONSTRUCTION SEQUENCE BELOW:
A. THE CONTRACTOR SHALL CONSTRUCT THE ENTIRE FACILITY (PERMANENT OUTLET STRUCTURE, DAM,
ETC.) WITH THE EXCEPTION OF THE INTERIOR FINE GRADING FOR THE FACILITY. THE INTERIOR FINE
GRADING WILL BE CONSTRUCTED ONCE THE EROSION CONTROL PHASE IS COMPLETE.
B. THE TEMPORARY DRAW DOWN RISER (OR SKIMMER) SHALL BE CONNECTED TO THE PERMANENT 6"0
DIP DRAIN PIPE.
C. ONCE THE UPSTREAM DRAINAGE AREA IS STABILIZED AND THE EROSION CONTROL INSPECTOR
APPROVES THE REMOVAL OF THE SEDIMENT BASIN, THE CONTRACTOR SHALL REMOVE THE TEMPORARY
DRAW DOWN RISER (OR SKIMMER) AND CLEAN OUT THE BASIN. ALL SEDIMENT, TRASH, ETC. SHALL BE
DISPOSED OF PROPERLY (I.E. - PLACED IN A LANDFILL) AND NOT STOCKPILED IN AN AREA WHERE WATER
QUALITY COULD BE ADVERSELY AFFECTED.
D. IT IS ANTICIPATED THAT DEWATERING WILL BE NECESSARY FOR CLEANING OUT THE BASIN AND
PROPOSED INTERIOR GRADING. THEREFORE, THE CONTRACTOR SHALL FURNISH, INSTALL, OPERATE, AND
MAINTAIN ANY PUMPING EQUIPMENT, ETC. NEEDED FOR REMOVAL OF WATER FROM VARIOUS PARTS
OF THE STORMWATER CONTROL MEASURE SITE. THE CONTRACTOR SHALL KEEP THE WATER LEVEL
BELOW THE BOTTOM OF THE EXCAVATION / CONSTRUCTION AREAS. THE MANNER IN WHICH THE
WATER IS REMOVED SHALL BE SUCH THAT THE EXCAVATION BOTTOM AND SIDE SLOPES ARE STABLE,
WITH NO SEDIMENT DISCHARGED FROM THE SITE (I.E. PUMPED WATER SHALL BE DIRECTED TO AN
APPROVED EROSION CONTROL DEVICE SUCH AS A SEDIMENT FILTER BAG (ACF ENVIRONMENTAL), OR
ENGINEER APPROVED EQUIVALENT, PRIOR TO DISCHARGE).
E. ALL DEWATERING DURING THE CONVERSION SHALL BE DONE THROUGH THE USE OF A SEDIMENT FILTER
BAG WITH A FLOATING INTAKE THAT IS CONTINUOUSLY MONITORED DURING OPERATION.
F. ONCE THE BASIN IS CLEANED AND PUMPED OUT, AND ALL EROSION CONTROL DEVICES REMOVED, THE
CONTRACTOR SHALL CONSTRUCT THE INTERIOR GRADING SHOWN ON THIS SHEET.
G.ONCE THE GRADING IS COMPLETE, THE CONTRACTOR SHALL REQUEST AN ON -SITE INSPECTION AND AN
AS -BUILT SURVEY PRIOR TO INSTALLATION OF THE STORMWATER CONTROL MEASURE PLANTS. IF THE
CONTRACTOR PLANTS THE PROPOSED VEGETATION PRIOR TO AN AS -BUILT SURVEY (AND SUBSEQUENT
APPROVAL), ANY CHANGES TO THE GRADING / RE -PLANTING OF PLANTS WILL BE AT THE CONTRACTOR'S
EXPENSE.
H. ONCE THE ENGINEER HAS APPROVED THE AS -BUILT GRADING, THE CONTRACTOR SHALL PLANT THE
PROPOSED STORMWATER CONTROL MEASURE PLANTS SHOWN ON THE LANDSCAPE PLAN FOR THE
FACILITY. AFTER COMPLETION OF THE PLANTING, THE LANDSCAPE CONTRACTOR SHALL PROVIDE A
LETTER TO THE ENGINEER CERTIFYING THAT THE PLANTS HAVE BEEN INSTALLED PER THE APPROVED
STORMWATER CONTROL MEASURE PLANTING PLAN.
